Pattern of Injury in Motorcycle Road Traffic Accidents Patients Treated in RSUP dr. Sardjito 2017

Latar belakang: Cedera dan kematian dari kecelakaan lalu-lintas adalah masalah kesehatan yang serius. Sebagian besar dari cedera dan kematian berasal dari negara dengan pendapatan rendah dan menengah, di mana sepeda motor digunakan sebagai moda transportasi utama. Indonesia dengan 71,68% dari total kecelakaan lalu-lintas melibatkan sepeda motor, maka penelitian mengenai pola perlukaan pada kecelakaan sepeda motor dan juga hubungannya dengan faktor risiko tertentu adalah suatu hal yang penting. Tujuan: Untuk mempelajari pola perlukaan pada kecelakaan sepeda motor dan korelasinya terhadap faktor risiko tertentu. Metode: Penelitian analitik observasional dengan desain penelitian transversal telah dilakukan pada Juni 2019, menganalisa total 96 sampel dari pasien kecelakaan sepeda motor di RSUP dr. Sardjito pada tahun 2017. Hasil: Dari seluruh pasien, cedera tungkai atas terdapat pada 44 pasien, cedera tungkai bawah dialami 40 pasien, dan 40 lainnya mengalami cedera kepala dan leher. Fraktur adalah jenis cedera paling umum yang dialami 45 dari seluruh pasien Sebagian besar pasien, yaitu sejumlah 55,2% mengalami cedera yang tergolong sebagai cedera berat. Terdapat korelasi antara keparahan cedera yang terjadi dengan jenis kelamin, penggunaan helm, dan jenis jalan di mana kecelakan terjadi dengan nilai p sebesar 0,034; 0,008; and 0,041. Juga terdapat korelasi antara penggunaan helm dan adanya cedera kepala berat dengan nilai p sebesar 0,001. Kesimpulan: Cedera dengan prevalensi terbanyak adalah fraktur, sebagian besar terjadi di tungkai atas dengan cedera kepala dan leher dan cedera tungkai bawah terbanyak berikutnya. Sebagian besar pasient mengalami cedera berat. Terdapat korelasi antara keparahan cedera dengan jenis kelamin, penggunaan helm, dan jenis jalan di mana kecelakaan terjadi. Juga terdapat korelasi antara adanya cedera kepala berat dengan penggunaan helm.

Background: Injuries and deaths from road traffic accidents are a major public health problem. Most of injuries and deaths are from low- and middle- income countries where motorcycles are used as significant amount of transport. In Indonesia where 71,68% of road traffic accidents involve motorcycles, it is important to study the resulting pattern of injury from motorcycle road traffic accidents and their correlations with certain risk factors to help identify occurring pattern of injury, to help in prevention of fatal injuries and to provide better medical care. Objective: To study the pattern of injury occurred in motorcycle road traffic accidents and its correlations with certain risk factors. Methods: Observative analytical study with cross-sectional design was carried out in June 2019, analyzing total of 96 samples from patients suffering from motorcycle road traffic injuries treated in RSUP dr. Sardjito during the year 2017. Result: From all patients, 44 had upper limb injuries present, lower limb injuries were present in 40 patients, and head and neck injuries were also present in 40 patients. Fractures were the most prevalent type of injury with 45 of all patients suffered from it. Most of the patients had injuries categorized as severe injury at 55,2% of all patients. There are correlations between resulting extent of injury with sex, helmet usage, and types of road in which accident occurred with p-value of 0,034; 0,008; and 0,041; respectively. There is also a correlation between severe head injuries and helmet usage with p-value of 0,001. Conclusion: Most prevalent injury is fracture, mostly happened on upper limbs with head and neck injury and lower limbs closely follow and most patients were categorized as having severe injury. There are correlations between extent of injury with sex, helmet usage, and types of road in which accidents occurred, and also between helmet usage and presence of severe head injury.
