PELAKSANAAN PENGODEAN DIAGNOSIS KASUS DIABETES MELLITUS PASIEN JAMINAN KESEHATAN NASIONAL (JKN) DI RSU PKU MUHAMMADIYAH BANTUL

Latar belakang : Petugas coding harus mampu menetapkan kode diagnosis dengan tepat sesuai klasifikasi yang diberlakukan di Indonesia (ICD-10) tentang diagnosis dalam pelayanan dan manajemen kesehatan. Berdasarkan hasil wawancara peneliti, pengodean diagnosis pada kasus diabetes mellitus masih terdapat permasalahan adanya ketidaktepatan kode berdasarkan ICD-10. Tujuan : Mendeskripsikan pelaksanaan pengodean diagnosis kasus diabetes mellitus pada pasien rawat inap JKN di RSU PKU Muhammadiyah Bantul. Metode : Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan rancangan penelitian fenomenologis. Subjek penelitian terdiri dari seluruh petugas coding (coder) yang berjumlah empat orang. Objek penelitian yaitu berkas rekam medis pasien rawat inap JKN kasus diabetes mellitus. Teknik pengambilan data yaitu wawancara, observasi, dan studi dokumentasi. Hasil : Pelaksanaan pengodean diagnosis pasien rawat inap di RSU PKU Muhammadiyah Bantul dilakukan oleh empat orang petugas coding, proses penentuan kode dilakukan secara komputerisasi menggunakan program pengodean yang terdapat di komputer. Tingkat ketepatan kode diagnosis kasus diabetes mellitus sebesar 46 (50%) tepat sesuai ICD-10. Upaya meningkatkan ketepatan kode yang sudah dijalankan dengan baik diantaranya mereview berkas sebelum menentukan kode dan menambah pengetahuan petugas terhadap aturan dalam pengodean diagnosis, sedangkan upaya konfirmasi kepada dokter untuk diagnosis yang belum jelas/tidak terbaca belum dilaksanakan dengan optimal. Kesimpulan : Pelaksanaan pengodean diagnosis kasus diabetes mellitus berdasarkan ICD-10 sudah baik namun untuk ketepatan kode masih perlu ditingkatkan. Sebaiknya dilakukan perbaikan seperti melakukan pembuatan&revisi SPO pengodean diagnosis, melakukan evaluasi/audit kode diagnosis, serta meningkatkan komunikasi yang baik antar profesional kesehatan.

Background : Coding officers (coder) must be able to correctly determine the diagnosis code according to the classification applied in Indonesia (ICD-10) about diagnosis in health services and management. Based on the results of interview researchers, the coding of diagnoses in cases of diabetes mellitus still has problems with the inaccuracy of the code based on the ICD-10. Objective : Describe the implementation of the coding diagnosis of diabetes mellitus cases in JKN hospitalized patients at PKU Muhammadiyah General Hospital Bantul. Methods : The type of research used is descriptive research with a qualitative approach and phenomenological research design. The subject of the study consisted of all coding officers totaling four people. The object of the research is the medical record file for inpatients of JKN in cases of diabetes mellitus. Data collection techniques are interviews, observation, and documentation studies. Results : The implementation of coding the diagnosis of inpatients at PKU Muhammadiyah Hospital in Bantul was carried out by four coder, the process of determining the code was done using computerized coding programs found on the computer. The level of accuracy of the diagnosis code for diabetes mellitus cases is 46 (50%) according to ICD-10. Efforts to improve the accuracy of the code that has been run well include reviewing the file before determining the code and increasing the knowledge of the officer on the rules in coding the diagnosis, while efforts to confirm to the doctor for an unclear / illegible diagnosis have not been implemented optimally. Conclusion : The implementation of the coding diagnosis of diabetes mellitus cases based on the ICD-10 has been good but for the accuracy of the code still needs to be improved. Improvements should be made, such as making&revising SPO coding diagnosis, evaluating diagnosis/audit codes, and improving good communication between health professionals.
