Faktor Ketidaklengkapan Pendokumentasian Resume Medis di Rumah Sakit Nur Hidayah

Latar Belakang: Resume medis adalah ringkasan seluruh perawatan dan pengobatan yang dilakukan oleh dokter kepada pasien. Berdasarkan studi pendahuluan pada bulan April-Mei 2019 di RS Nur Hidayah, hasil analisis 25 resume medis didapatkan persentase kelengkapan pada aspek identifikasi sebesar 41%, laporan penting 77,85%, autentikasi 74,67%, dan pendokumentasian yang benar 100%. Hasil analisis tersebut menunjukkan bahwa masih terdapat resume medis yang tidak lengkap. Hal ini tidak sesuai dengan pelayanan minimal rumah sakit dan standar akreditasi yang menyebutkan standar indikator kelengkapan pengisian rekam medis yaitu 100%. RS Nur Hidayah pada bulan September 2019 akan dilaksanakan akreditasi berdasarkan SNARS sehingga penting dilakukan evaluasi terhadap permasalahan itu untuk mempersiapkan reakreditasi. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran umum kelengkapan lembar resume medis di RS Nur Hidayah. Metode: Jenis penelitian yang digunakan adalah penelitian kualitatif dengan rancangan fenomenologi. Hasil: Resume medis masih tidak lengkap walaupun sudah dilengkapi. Faktor penyebab ketidaklengkapan resume yaitu dokter terburu-buru mengisi resume medis, ketidakdisiplinan dokter, perawat lupa mengingatkan dokter, tidak ada SPO tentang pengisian resume medis dan pengendalian rekam medis tidak lengkap, dan tidak ada penggunaan label identitas pasien. Upaya yang telah dilakukan untuk mengurangi ketidaklengkapan tersebut yaitu meningkatkan kedisiplinan dokter, meningkatkan koordinasi antartenaga kesehatan, menggunakan kartu kendali, dan metode PDSA. Kesimpulan: Pelaksanaan pengisian resume medis di RS Nur Hidayah sudah sesuai dengan aturan hanya saja perlu ditingkatkan lagi terkait kelengkapan resume medis. Faktor penyebab ketidaklengkapan resume medis yaitu SDM, metode, dan mesin. Upaya pengendalian ketidaklengkapan resume medis perlu dibuat kebijakan/SPO.

Background: Discharge summary is a summary of all treatments and treatments performed by doctors to patients. Method: The type of research used is qualitative research with phenomenological design. Results: The discharge summary is still incomplete even though it has been completed. Factors that cause incompleteness of discharge summary are doctors hurrying to fill out medical resumes, indiscipline of doctors, nurses forget to remind doctors, no SPO about filling out medical resumes and incomplete medical record control, and no use of patient identity labels. Efforts that have been made to reduce these incompleteness include improving doctor discipline, increasing coordination between health staff, using control cards, and PDSA methods. Conclusion: The implementation of filling out a discharge summary at Nur Hidayah Hospital is in accordance with the rules, but only needs to be improved again regarding the completeness of medical resumes. Factors that cause incompleteness of medical resumes are HR, methods, and machines. Efforts to control incomplete medical resumes need to be made policy / SOP.
