Identifikasi Potensi Medication Error Dan Peranan Apoteker Dalam Pencegahannya Serta Penetapan Beyond Use Date Resep Racikan Di Apotek Kota Yogyakarta Bagian Timur

Peresepan obat racikan oleh dokter masih diberikan. Resep racikan ini berpotensi menimbulkan permasalahan dan penyebab terjadinya medication error. Skrining resep yang dilakukan apoteker dapat mencegah potensi terjadinya medication error. Selain itu, Beyond Use Date dari sediaan racikan juga perlu ditetapkan oleh apoteker untuk mengetahui batasan waktu penggunaan oleh pasien. Penelitian ini bertujuan untuk mengetahui jumlah resep racikan yang berpotensi menimbulkan medication error, mengetahui peran apoteker dalam mencegah medication error serta dalam menetapkan Beyond Use Date sediaan racikan di apotek wilayah Kota Yogyakarta bagian timur. Penelitian ini merupakan penelitian cross sectional. Pengambilan data dilakukan secara retrospektif dengan metode purposive sampling dari resep racikan pada 3 apotek yang berada di wilayah Kota Yogyakarta bagian timur periode Juli sampai Desember 2018 untuk mengetahui potensi medication error. Dilakukan pula pengambilan data dengan wawancara kepada apoteker secara concurrent untuk mengetahui peran apoteker dalam mencegah medication error serta dalam menetapkan Beyond Use Date sediaan racikan. Data kemudian dianalisis secara deskriptif. Hasil identifikasi medication error dari jumlah sampel sebanyak 402 lembar resep racikan menunjukkan bahwa sebanyak 378 lembar resep atau sebesar 94,03% terdapat potensi error. Potensi error yang ditemukan pada aspek administratif sebanyak 65, 17% (262 error), aspek farmasetik sebanyak 25,37% (102 error) dan aspek klinis sebanyak 84,58% (340 error) (n=402). Peran apoteker dalam mencegah dan mengatasi permasalahan tersebut yaitu dengan melakukan konfirmasi kembali kepada pasien terkait error dalam aspek administrasi, melakukan konfirmasi kepada dokter terkait error aspek farmasetik dan klinis serta melakukan studi literatur. Peran apoteker dalam menetapkan beyond use date yaitu dengan melakukan studi literatur serta memberikan edukasi dan informasi kepada pasien terkait batas penggunaan obat berdasarkan stabilitas fisik sediaan racikan.

Extemporaneous prescription is still given by the doctors. This prescription potentially provokes problems and the cause of medication error. The prescription screening done by pharmacist could prevent the potency of medication error. In addition, beyond used date from the extemporaneous compounding also needs to be determined by the pharmacist to know when that prescription should no longer be used by the patient. This study aims to determine the number of extemporaneous prescriptions that could potentially lead to the medication errors, to find out the role of the pharmacist in preventing medication errors and to establish beyond use date in extemporaneous prescription. This study was conducted with a cross sectional design. The data retrieval was carried out retrospectively with a purposive sampling method from extemporaneous prescriptions at 3 pharmacies in the eastern region of Yogyakarta city during the period July to December 2018 to determine the potential medication error. In addition, this study also collected data by interviewing pharmacists concurrently to find out the role of the pharmacist in preventing medication error and establishing beyond use date for extemporaneous prescription. The data was then analyzed through descriptive method. The results from the identification of medication errors in extemporaneous prescription showed that errors found in the administrative aspects was 65,17% (262 errors), pharmaceuticals aspects was 25,37% (102 errors) and clinical aspects was 84,58% (340 errors) (n = 402). In preventing and solving these problems, the pharmacists have been doing the confirmation to the patient regarding errors in the administrative aspects, confirming to the doctor regarding errors in pharmaceuticals and clinical aspects and also conducting literature studies. In establishing the beyond use date, the pharmacists have been conducting literature studies, providing education and information to patients regarding the limits of drug use based on the changes in the physical stability of the preparation.
