Identifikasi Potensi Medication Error dan Peran Apoteker pada Pelayanan Resep Racikan di Apotek Wilayah Kota Yogyakarta Bagian Utara

Peresepan sediaan racikan masih dilakukan dokter untuk memperoleh pengobatan sesuai dengan kondisi pasien. Kejadian medication error masih sering ditemukan khususnya dalam resep racikan. Sehingga adanya evaluasi terkait potensi medication error dan penentuan beyond use date dalam pelayanan resep racikan di apotek perlu dilakukan. Tujuan dari penelitian ini adalah untuk mengidentifikasi potensi kejadian medication error pada resep racikan dan mengetahui peran apoteker dalam mengatasinya serta menetapkan beyond use date sediaan racikan. Penelitian ini merupakan penelitian cross sectional. Pengambilan data dilakukan secara retrospektif dengan metode purposive sampling dari lembar resep di 3 apotek di wilayah Kota Yogyakarta bagian utara. Lembar resep yang diambil sebagai sampel yaitu yang terdapat resep racikan pada periode bulan Juli sampai Desember 2018. Data dikumpulkan menggunakan lembar pengumpul data untuk mengetahui potensi medication error. Wawancara dengan apoteker dilakukan secara concurrent menggunakan lembar panduan wawancara yang telah dibuat untuk mengetahui peran apoteker dalam mencegah medication error dan menetapkan beyond use date sediaan racikan. Data resep dianalisis secara deskripstif dan hasil wawancara disajikan dalam bentuk narasi. Total sampel yang diperoleh sebanyak 400 lembar dari 3 apotek di wilayah Kota Yogyakarta bagian utara. Potensi medication error yang ditemukan pada aspek administratif sebanyak 92,5%, aspek farmasetis sebanyak 5% dan aspek klinis sebanyak 23,75%. Peran apoteker dalam mengatasi potensi medication error adalah melakukan konfirmasi kepada pasien terkait error pada aspek administratif, melakukan konfirmasi kepada dokter terkait error pada aspek farmasetis dan klinis. Peran apoteker dalam menetapkan beyond use date yaitu melakukan penetapan berdasarkan pemahaman yang dimiliki.

Concoction prescribing by doctors to provide treatment that is appropriate to the patient's condition. The occurence of medication errors in concoction recipes is still common. Therefore an evaluation related to the potential medication error is needed and the determination of beyond use date in concoction prescription services at the pharmacy. The purpose of this study was to identify the potential occurrence of medication errors in concoction recipes and to know the role of the pharmacist in preventing and determining beyond use date. This research was conducted a cross sectional design. The data retrieval was carried out retrospectively with a purposive sampling method from the recipe sheet in 3 pharmacies in the northern part of Yogyakarta City. The recipe sheet taken as a sample was containing a concoction recipe in the period from July to December 2018. Data was collected using data collection sheet to determine the potential medication error. Interview with pharmacists concurrently using interview guide sheets to determine the role of the pharmacist in preventing medication errors and setting out the use of concoction preparations. The recipe data was analyzed descriptively and the results of interviews were presented in narrative form. Total samples of concoction recipes were obtained 400 sheets from 3 pharmacies in the northern part of Yogyakarta City. The potential medication error in the prescription were 92,5% on the administrative aspect, 5% on the pharmaceutical aspect and 23,75% on the clinical aspect. The role of the pharmacist in preventing potential medication errors was by confirming to the patient regarding errors in the administrative aspects, confirming to the doctor regarding errors in pharmacetic and clinical aspects. Pharmacists have a role in determining beyond use date based on understanding and experience during the work period.
