Identifikasi Potensi Medication Error serta Peranan Apoteker Dalam Pengatasannya dan Penetapan Beyond Used Date Sediaan Racikan Di Apotek Kecamatan Depok, Sleman

Sediaan racikan banyak digunakan pada pelayanan resep di Indonesia. Namun keamanannya belum terjamin karena ditemukan kejadian medication error. Apoteker memegang peranan penting dalam menjaga keselamatan pasien dari pelayanan resep yang irrasional. Penetapan beyond used date dengan tepat penting dilakukan untuk meningkatkan aspek keamanan pasien. Penelitian ini bertujuan untuk mengetahui persentase potensi medication error, peranan apoteker dalam mencegah medication error, dan menetapkan beyond used date. Penelitian dilakukan dengan rancangan cross sectional. Identifikasi potensi medication error secara retrospektif menggunakan data resep Bulan Juli sampai Desember 2018 dari 3 apotek di Kecamatan Depok, Sleman. Pengambilan sampel secara purposive sampling. Data dikumpulkan menggunakan lembar pengumpul data untuk mencatat potensi kejadian medication error dari dokumen resep apotek. Wawancara dengan apoteker dilakukan untuk mengetahui cara apoteker mencegah medication error dan menentukan beyond used date. Wawancara dilakukan secara concurrent berdasarkan pedoman wawancara yang telah dibuat. Data potensi kejadian medication error disajikan dalam bentuk persentase, sedangkan hasil wawancara akan dipaparkan dalam bentuk narasi. Pada penelitian ini dilakukan analisis terhadap 402 lembar resep racikan. Semua resep yang diteliti memiliki 1-2 informasi yang kurang lengkap sehingga aspek administratif resep tidak lengkap. Sebanyak 146 (36,61%) resep pada aspek farmasetis, dan 251 (62,43%) pada aspek klinis. Apoteker sudah berperan dalam mencegah kejadian medication error dengan menanyakan informasi kepada pasien untuk mengatasi potensi error pada aspek administrasi, konfirmasi ke dokter untuk mengatasi potensi error pada aspek farmasetis, studi literatur, menghitung dosis, dan diskusi ke dokter untuk mengatasi potensi error pada aspek klinis. Apoteker menetapkan beyond used date sediaan racikan 14-30 hari tergantung kepada bentuk sediaan. Apoteker menetapkan beyond used date berdasarkan pengalaman, belum menggunakan USP sebagai acuan.

Extemporanous drug preparation are widely used in community pharmacy in Indonesia. The safety aspect is not guaranteed because medication errors occurs. Pharmacists play an important role in saving patients from irrational prescription services. If the determination of beyond used date used done correctly, patient safety aspects will improve. This study aims to determine the percentage of potential medication errors, what pharmacits role to prevent medication errors, and determine beyond used date. The study was conducted with a cross sectional design. Identification of potential retrospective medication errors using prescription data from July to December 2018 from 3 community pharmacies in Depok, Sleman District. Sampling by purposive sampling. Data was collected using a data collection sheet to record the potential occurrence of medication errors from pharmacy prescription documents. Interviews with pharmacists were conducted to find out how pharmacists prevent medication errors and determine beyond used date. Interviews are conducted concurrently based on the interview guidelines that have been made. Data on the potential occurrence of medication errors are presented in the form of percentages, while the results of the interviews will be presented in narrative form. In this study an analysis of 402 extemporanous recipes. All recipes have 1-2 information that incomplete which makes the administrative aspect incomplete. The percentage of medication errors was 146 (36.61%) prescriptions on pharmaceutical aspects and 251 (62.43%) on clinical aspects. Pharmacists have a role in preventing the occurrence of medication errors by asking patients for the information to overcome error potention in pharmacetutical aspect, confirmation to prescriber confirmation do literature study, calculate the dosage, and discussion with prescriber to overcome error potention in clinical aspect. Pharmacists determines beyond used date 14-30 days depends of the dosage form. Pharmacist have not used official references in determining beyond used date, they determines it by their experience.
