EVALUASI PELAKSANAAN PENYUSUTAN BERKAS REKAM MEDIS RAWAT JALAN DAN PENYIMPANAN BERKAS REKAM MEDIS INAKTIF DI PUSKESMAS SENTOLO II KULON PROGO

Latar Belakang: Rak penyimpanan rekam medis aktif di Puskesmas Sentolo II sudah penuh, sehingga Puskesmas Sentolo II melakukan penyusutan. Penyusutan berkas rekam medis rawat jalan merupakan penyusutan yang dilakukan pertama kali. Jangka waktu penyimpanan rekam medis adalah tiga tahun. Saat ini Puskesmas Sentolo II sedang melakukan persiapan reakreditasi, sehingga harus menyiapkan dokumen terkait penilaian akreditasi salah satunya adalah dokumen kriteria 8.4.3 elemen penilaian ke tiga yaitu terkait penyimpanan rekam medis dan masa retensi. Tujuan: Mengevaluasi proses penyusutan berkam rekam medis rawat jalan dan penyimpanan berkas rekam medis inaktif di Puskesmas Sentolo II. Metode:Jenis penelitian ini deskriptif dengan pendekatan kualitatif dan rancangan penelitian fenomologi. Teknik pengambilan data adalah studi dokumentasi, observasi dan wawancara. Uji keabsahan data menggunakan triangulasi sumber dan teknik. Hasil: Pelaksanaan penyusutan berpedoman pada KAK Retensi dan dilaksanakan selama 2 bulan yaitu pada Bulan Februari-Maret 2018. Berkas rekam medis yang dipilah adalah berkas rekam medis dengan kunjungan tiga tahun terakhir sedangkan dalam SOP berkas rekam medis dikatakan inaktif apabila kunjungan terakhir lebih dari lima tahun. Penyimpanan berkas rekam medis dilakukan dalam rak penyimpanan inaktif dengan sistem TDF. Sebelum terdapat rak, penyimpaan dilakuakan dengan ditumpuk dan diikat menggunakan tali. Terdapat hambatan yang menyebabkan pelaksanaan penyusutan dan penyimpanan berjalan tidak maksimal. Kesimpulan: Pelaksanaan penyusutan rekam medis tidak sesuai dengan aturan dalam SOP akan tetapi sesuai dalam KAK. Belum ada regulasi khusus penyimpanan berkas rekam medis inaktif. Hambatan pelaksanaan penyusutan dan penyimpanan yaitu SDM, tidak ada regulasi yang mengatur jangka waktu penyimpanan rekam medis inaktif dan label nomor rak penyimpanan. Background: Storage shelves of active medical records at Sentolo II Community Health Center was full, so Sentolo II Health Center did depreciation. Depreciation of outpatient medical records is the first depreciation. The period of retention of medical records is 3 years. Currently Sentolo II Health Center is preparing for re-accreditation, so Sentolo II Health Center must prepare documents related to accreditation assessment, one of which is criteria 8.4.3, the third element of assessment are the storage of medical records and retention period Objective: To evaluate the depreciation of outpatient medical records and storage of inactive medical record at Sentolo II Health Center. Method: This type of research was descriptive qualitative approach and phenomenological. The data collection techniques were the study of documentation, observation and interview. The validity test of data used source and techniques triangulation. Result: Implementation of depreciation was based on Retention TOR and was held for 2 months in February-March 2018. The disaggregated medical records is medical records with the last 3 years visit while in the SOP medical record file is said to be inactive if the last visit is more than 5 years. Medical record file storage is carried out in an inactive storage rack with a TDF system. Before there is a shelf, the storage is done by stacking and tied using a rope. There are obstacles that cause the implementation of depreciation and storage is not maximal. Conclusion: The implementation of medical records depreciation is not in accordance with the rules in the SOP but is appropriate in the TOR. There is no specific regulation regarding the storage of inactive medical records. Obstacles in the depreciation and storage are HR, there is no regulation that regulate the period of storage of inactive medical records and there are no labels of rack.
