PERANCANGAN MAP REKAM MEDIS DAN TRACER DI PUSKESMAS KOTA MUNGKID KABUPATEN MAGELANG

Latar belakang: Map rekam medis penting untuk menjaga keutuhan formulir rekam medis dari kerusakan dan tercecer. Puskesmas Kota Mungkid mengunakan map rekam medis berjenis stof map berbahan manila tipis dan mudah robek. Sementara itu, tracer dapat digunakan sebagai pengendali berkas rekam medis yang keluar dari rak. Tracer di Puskesmas Kota Mungkid belum tercantum informasi mengenai nomor rekam medis, nama pasien, tujuan keluar, dan tanggal keluar. Berdasarkan hal tersebut perancang memiliki ide untuk membuat rancangan ulang map rekam medis dan tracer sesuai standar dan kebutuhan pengguna. Tujuan: Merancang ulang map rekam medis dan tracer sesuai ketentuan dan kebutuhan Puskesmas Kota Mungkid. Metode: Perancangan map rekam medis dan tracer dilaksanakan di Puskesmas Kota Mungkid pada bulan Februari 2019. Tahapan perancangan terdiri dari penentuan ide, identifikasi kebutuhan, perancangan, presentasi, pemilihan desain, uji coba, dan hasil akhir. Hasil: Perancangan ini menghasilkan 2 alternatif desain untuk map rekam medis dan tracer. Setelah proses presentasi hasil rancangan, terpilih map rekam medis alternatif 1 dan tracer alternatif 1. Hasil akhir berupa rancangan map rekam medis dan tracer yang telah diuji coba oleh petugas dengan beberapa perbaikan. Kesimpulan: Dihasilkan rancangan baru map rekam medis dan tracer sesuai ketentuan dan kebutuhan pengguna. Sebaiknya Puskesmas Kota Mungkid menerapkan rancangan tersebut agar rekam medis dapat terlindungi dan terkendali dengan baik.

Background: Medical record folder is important to maintain the integrity of the medical record form from damage and scatter. Puskesmas Kota Mungkid uses a stof map medical record folder made from thin Manila that easily torn. Meanwhile, the tracer can be used as controller for the use of medical record. Tracer in Puskesmas Kota Mungkid has not listed information about the medical record number, patient name, destination, and date of discharge. Therefore, the designer has the idea to redesign medical record folder and tracer according to the standards and needs of the user. Objective: Redesigning medical record folder and tracer according to the needs of Puskesmas Kota Mungkid and appropriate with the standard. Methods: The design of medical record folder and tracer implemented at Puskesmas Kota Mungkid in February 2019. The design phase consisted of idea determination, needs identification, design, presentation, design selection, trial, and final results. Result: This design produces 2 alternative designs for medical record folder and tracer. After the presentation, design of medical record folder alternative 1 and tracer alternative 1 were selected. The final result is the selected medical record folder and tracer design which have had tested by the employees with some improvements. Conclusion: A new medical record folder and tracer design are produced according to the standards and the needs of users. It is better for Puskesmas Kota Mungkid to implement the design so that the medical record can be well protected and controlled.
