STRATEGI PELAKSANAAN PERCEPATAN PROGRAM JAMINAN KESEHATAN NASIONAL DALAM MENCAPAI UNIVERSAL HEALTH COVERAGE (UHC) DI KOTA YOGYAKARTA

Jaminan Kesehatan Nasional (JKN) menjadi amanah wajib di bidang kesehatan yang harus diimplementasikan oleh seluruh Kabupaten/Kota di Indonesia. Target program JKN adalah mencapai Universal Health Coverage (UHC) di Tahun 2019, dalam artian bahwa lebih dari 95% penduduk Indonesia terdaftar menjadi peserta program JKN dan didukung oleh fasilitas kesehatan yang memadai berdasarkan rasio kecukupan. Namun tidak sedikit daerah di Indonesia yang belum berhasil mencapai UHC hingga target waktu yang ditentukan. Kota Yogyakarta menjadi salah satu dari dua Kabupaten/Kota di DIY dan Jawa Tengah yang berhasil mencapai UHC di tahun 2018. Sehingga penelitian ini bertujuan untuk (1) menjelaskan turunan strategi Pemerintah Kota Yogyakarta mendukung proses implementasi JKN dalam upaya mencapai Universal Health Coverage di Tahun 2019 dan (2) mengidentifikasi faktor-faktor yang mempengaruhi proses implementasi JKN di Kota Yogyakarta. Metode penelitian yang digunakan adalah pendekatan analisis kualitatif deskriptif, dengan lokasi penelitian di lingkungan Kota Yogyakarta. Hasil penelitian menunjukan bahwa turunan strategi dan arah kebijakan kesehatan yang dirumuskan Pemerintah Kota Yogyakarta telah sejalan dengan proses implementasi JKN dan mendukung tujuan nasional mencapai Universal Health Coverage (UHC) JKN di Tahun 2019. Secara inisiatif, Pemerintah Kota Yogyakarta mengemas dengan ringkas dalam “12 Pokok Pikiran Penguatan Kota Yogyakarta” yang berisi intisari seluruh arah kebijakan yang ditetapkan. Capaian UHC di Kota Yogyakarta telah didukung oleh lingkungan implementasi yang kondusif antara lain kepentingan dan strategi aktor yang terlibat, karakteristik rezim yang sedang berkuasa dan tingkat kepatuhan dan respon kelompok sasaran (masyarakat). Dukungan implementasi JKN juga diperoleh dari upaya-upaya yang telah dilakukan Pemerintah Kota Yogyakarta dalam tiga dimensi JKN yaitu (1) program perluasan cakupan kepesertaan JKN, (2) menjaga kecukupan anggaran urusan kesehatan sesuai ketentuan (compliance), dan (3) dukungan Fasilitas Kesehatan Tingkat Pertama (FKTP) JKN melalui akreditasi dan penerapan pengelolaan BLUD.

Jaminan Kesehatan Nasional (JKN) is a mandatory program in the health sector that should be implemented by all districts/cities in Indonesia. The target of the JKN program is to achieve Universal Health Coverage (UHC) in 2019, at the sense that more than 95% of the Indonesian population are registered as membership in the JKN program and are supported by adequate health facilities based on the adequacy ratio. But several regions in Indonesia have not reached the UHC yet until the specified time target. Yogyakarta City is one of two regencies/cities in DIY and Central Java that have succeeded in reaching UHC in 2018. So this study aims to (1) explain the derivative strategy of the Government of Yogyakarta City to support the JKN implementation process in achieving Universal Health Coverage in 2019 and (2) identify the factors that influence the JKN implementation process in Yogyakarta City. The research method used is a descriptive qualitative analysis approach, with research locations in the Yogyakarta City environment. The results showed that the derivative of the strategy and direction of the health policy formulated by the Government of Yogyakarta City was in line with the JKN implementation process and supported the national goal of achieving Universal Health Coverage (UHC) JKN in 2019. In initiatively, the Government of Yogyakarta City summarize briefly "12 Pokok Pikiran Penguatan Kota Yogyakarta" which contained the essence of the direction of the policy set. The achievements of UHC in the Yogyakarta City have been supported by a conducive implementation environment including the interests and strategies of the actors involved, the characteristics of the ruling regime and the level of compliance and response of the target group (community). Support for JKN implementation was also obtained from the efforts made by the Government of Yogyakarta City in the three dimensions of JKN, that are (1) the program to expand the coverage of JKN membership, (2) maintaining the adequacy of the health affairs budget, and (3) support of First Level Health Facility (FKTP) through accreditation and application of BLUD management.
