FAKTOR PENYEBAB DAN DAMPAK KETIDAKTERISIAN KODE DIAGNOSIS PADA LEMBAR RAWAT JALAN DI PUSKESMAS SRUWENG KEBUMEN

Latar Belakang: Dalam menyelenggarakan pelayanan, puskesmas mempunyai bagian penting dalam proses kerjanya yaitu rekam medis, termasuk melakukan klasifikasi dan kodefikasi terhadap diagnosis penyakit. Berdasarkan hasil Praktik Kerja Lapangan pada Bulan Juli 2018 di Puskesmas Kebumen III didapatkan 52 dari 100 lembar rawat jalan yang kode diagnosisnya tidak terisi dan hasil studi pendahuluan yang dilakukan pada Bulan Februari di Puskesmas Sruweng dari 86 lembar rawat jalan yang diamati, terdapat 70 lembar yang kode diagnosisnya tidak terisi. Tujuan: Mendeskripsikan pelaksanaan pengodean diagnosis pada lembar rawat jalan di Puskesmas Sruweng Kebumen, mengidentifikasi faktor penyebab ketidakterisian kode diagnosis pada lembar rawat jalan di Puskesmas Sruweng Kebumen, mengidentifikasi dampak ketidakterisian kode diagnosis pada lembar rawat jalan di Puskesmas Sruweng Kebumen, dan mengetahui upaya yang telah dilakukan puskesmas untuk mengatasi ketidakterisian kode diagnosis pada lembar rawat jalan di Puskesmas Sruweng Kebumen. Metode penelitian: Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif. Rancangan penelitian yang digunakan adalah fenomenologi. Subjek dari penelitian ini antara lain dokter, perawat, petugas rekam medis dan kepala puskesmas. Objek pada penelitian ini adalah kode diagnosis pada lembar rawat jalan. Teknik pengambilan data yang digunakan adalah wawancara, observasi, dan studi dokumentasi. Hasil: Proses pengodean di Puskesmas Sruweng Kebumen dilaksanakan oleh dokter dan perawat menggunakan buku pintar, berdasarkan ingatan dan mencari di internet. Faktor penyebab ketidakterisian kode diagnosis di Puskesmas Sruweng Kebumen berasal dari segi man, material dan method. Ketidakterisian kode diagnosis pada lembar rawat jalan di Puskesmas Sruweng Kebumen berdampak pada kelengkapan berkas rekam medis dan laporan puskesmas. Kesimpulan: Faktor penyebab ketidakterisian kode diagnosis di Puskesmas Sruweng Kebumen dianalisis berdasarkan 5M. Terdapat dampak yang ditimbulkan dari ketidakterisian kode diagnosis. Upaya ketidakterisian kode diagnosis sudah dilakukan, tetapi upaya tersebut belum maksimal.

Background: In organizing services, puskesmas has an important part in its work process, there is medical record, including conducting classification and codification of the diagnosis of the disease. Based on the results of the work practices in July 2018 at Puskesmas Kebumen III, 52 out of 100 outpatient sheets the diagnosis code was not filled and the results of a preliminary study on February 2019 at Puskesmas Sruweng Kebumen from 86 outpatient sheets were observed, there were 70 sheets whose diagnosis codes were not filled. Objective: To describe the implementation of diagnosis coding on outpatient sheets at Puskesmas Sruweng Kebumen, identify the causes of incompleteness of the diagnosis code on the outpatient sheet at Puskesmas Sruweng Kebumen, identify the impact of incomplete diagnosis codes on outpatient sheets at Puskesmas Sruweng Kebumen, and find out what efforts have been made to overcome the incompleteness of the diagnosis code on the outpatient sheet at Puskesmas Sruweng Kebumen. Methods: Type of this research is descriptive with a qualitative approach. The research design used phenomenology. The subjects of this research are doctors, nurses, medical record officer and Head of Puskesmas Sruweng Kebumen. The object of this research is diagnosis codes in outpatient sheets. The data collection techniques used interviews, observation, and documentation studies. Results: The coding process at Puskesmas Sruweng Kebumen was carried out by doctors and nurses using smart books, based on memory and searching the internet. The factors that cause the incompleteness of the diagnosis code at Puskesmas Sruweng Kebumen are derived from man, material and method. The incompleteness of the diagnosis code on the outpatient sheet at Puskesmas Sruweng Kebumen has an impact on the completeness of the medical record file and the puskesmas report. Conclusions: The factors that caused the incompletion of the diagnosis code at Puskesmas Sruweng Kebumen were analyzed based on 5M. There is an impact arising from the incompleteness of the diagnosis code. Efforts to fill in the diagnosis code have been made, but the effort has not been maximized.
