PERANCANGAN FORMULIR DAN MAP (FOLDER) REKAM MEDIS DI PUSKESMAS PALIYAN GUNUNGKIDUL

ABSTRAK Latar Belakang: Salah satu pelayanan medis tingkat pertama yang sangat dibutuhkan masyarakat adalah puskesmas. Dalam memberikan pelayanan kesehatan puskesmas memerlukan sarana penunjang yang memadai seperti penyelenggaraan rekam medis. Menurut Peraturan Menteri Kesehatan Republik Indonesia No. 269 tahun 2008 tentang rekam medis, rekam medis adalah berkas yang berisikan catatan dan dokumen tentang identitas pasien, pemeriksaan, pengobatan, tindakan dan pelayanan kepada pasien. Desain map rekam medis dan formulir rawat jalan di Puskesmas Paliyan belum sesuai dengan ketentuan Permenkes 269 tahun 2008. Belum teradapat formulir kesehatan ibu dan anak, formulir gigi dan informed consent yang dibutuhkun guna menunjang pelayanan. Tujuan: Merancang ulang map rekam medis dan formulir rawat jalan, merancang formulir kesehatan ibu dan anak, merancang formulir gigi dan merancang informed consent. Metode: Perancang melakukan studi pendahuluan dan menentukan untuk melakukan perancangan map dan formulir dengan landasan perancangan Permenkes 269 tahun 2008. Membuat desain awal dengan dua pilihan alternatif sesuai kebutuhan pengguna untuk diajukan kepada responden yang akhirnya dipilih salah satu dari dua alternatif serta memperbaiki desain yang terpilih untuk disetujui. Hasil: Desain Map Rekam Medis terpilih menggunakan kertas ivory 260 gram berwarna putih berukuran A3 sedangkan formulir rawat jalan, formulir kesehatan ibu anak, formulir gigi, dan formulir informed conset menggunakan kertas hvs 80 gram berwarna putih berukuran A4. Kesimpulan: Identifikasi map rekam medis dan formulir rawat jalan belum sesuai dengan ketentuan Permenkes 269 tahun 2008. Map rekam medis, formulir rawat jalan, formulir kesehatan ibu dan anak, formulir gigi, dan informed consent sudah dirancang dan sudah terpilih.

ABSTRACT Background: One of the first level medical sevices that needed by the community is a health center/clinic. In providing health services the health center/clinic need adequate supporting facilities such as administering medical records. According to the Republik of Indonesia Health Minister regulation number 269 of 2008 concerning medical records, medical record is a file containing notes and documents about patient identity, examination, treatment, action and service to patients. The medical record folder and outpatient form in the Puskesmas Paliyan are not yet in accordance with the provisions of Permenkes 269 of 2008. There is no maternal and child health form, dental health form, and informed consent needed to support services. Objective: Redesigning medical record folder and outpatient form, designing maternal and child form, designing dental health form and designing informed consent. Methods: The designer conducts a preliminary study and determines to design a medical record folder and form with the basis of design of the Permenkes 269 of 2008. Make an initial design with two alternative choiches according to user needs to be submitted to respondent who finally selected one of the two alternatives and improve the design chosen for approval. Results: Selected medical record folder design using 260 gram white ivory paper in A3 size while the outpatient form, maternal and child health form, dental health form and informed consent form using 80 gram white in A4 hvs paper. Conclusion: Identification of medical record folder and outpatient form not in accordance with the provisions of the Permenkes 269 of 2008. Medical record folder, outpatient form, maternal and child health form, dental health form and informed consent was designed and selected.
