INTISARI Terapi untuk Penyakit Diabetes Melitus (DM) memerlukan biaya yang tinggi. INA-CBGs merupakan sistem pembayaran kesehatan dengan sistem paket yang didasarkan pada diagnosis akhir dan prosedur yang diterima pasien. Dalam penerapannya, tidak jarang ditemukan bahwa terdapat perbedaan antara biaya riil dengan tarif INA-CBGs. Penelitian ini bertujuan untuk mengetahui besarnya biaya terapi, faktor yang mempengaruhi, serta kesesuaian biaya riil dengan tarif INA-CBGs pada pasien DM tipe 2 rawat jalan di RSUD Sleman D.I Yogyakarta. Penelitian ini merupakan penelitian analitik non eksperimental. Pengambilan data dilakukan secara retrospektif melalui catatan medis pasien dan rincian biaya perawatan pasien. Subjek penelitian adalah pasien DM tipe 2 peserta JKN yang melakukan rawat jalan pada periode September-November 2017 dengan kode INA-CBGs Q-5-44-0, Z-3-23-0, M-3-16-0, dan H-3-12-0. Data yang digunakan meliputi daftar biaya medis langsung berupa obat dan alat medis, biaya pemeriksaan dokter, dan biaya penunjang seperti biaya pemeriksaan laboratorium. Untuk mengetahui masing-masing faktor yang mempengaruhi biaya riil dan INA-CBGs dilakukan uji Kruskall wallis dan Mann-Whitney, sedangkan one sample test digunakan untuk mengetahui komparasi biaya riil dengan tarif INA-CBGs. Hasil penelitian menunjukkan bahwa pasien yang masuk kriteria inklusi yakni pasien yang berusia lebih dari 18 tahun dan pasien secara rutin melakukan pemeriksaan kesehatan minimal 3 bulan di RSUD Sleman Yogyakarta berjumlah 99 pasien dengan 330 episode kunjungan rawat jalan pada periode September-November 2017. Total biaya pasien penyakit DM berdasarkan perspektif rumah sakit dalam kurun waktu September hingga November 2017 adalah Rp. 151.665.050,00 dengan 84,45 % terbanyak biaya adalah untuk biaya obat dan barang medis. Usia, Jenis Kelamin, Jumlah Komplikasi, Lama menderita DM, BMI(Body Mass Index) dan Jenis Obat Antihiperglikemia yang digunakan tidak berbeda signifikan dengan total biaya riil diabetes melitus. Terdapat selisih negatif antara biaya riil dan tarif INA-CBGs sebesar Rp. -86.664.000 untuk total 99 pasien dengan jumlah 330 episode kunjungan rawat jalan pada masing-masing kode INA CBGs yakni Q-5-44-0, Z-323-0, M-3-16-0 dimana (p= <0,05) menunjukkan adanya signifikansi perbedaan antara biaya riil dengan tarif INA-CBGs. ABSTRACT Therapy for Diabetes Mellitus (DM) requires a high cost. INA-CBGs is a health payment system with a package system that is based on the final diagnosis and procedure received by the patient. In its application, it is not uncommon to find that there are differences between the real costs and the rates of INA-CBGs. This study aims to determine the magnitude of the cost of therapy, influencing factors, and the suitability of real costs with the rate of INA-CBGs in outpatient type 2 DM patients at RSUD Sleman D.I Yogyakarta. This study is a non-experimental analytical study. Data retrieval is done retrospectively through patient medical records and details of patient care costs. The research subjects were type 2 DM patients JKN participants who did outpatient treatment in the period September-November 2017 with the code INA-CBGs Q-5-44-0, Z-3-23-0, M-3-16-0, and H -3-12-0. The data used includes a list of direct medical costs in the form of drugs and medical devices, medical examination fees, and support costs such as laboratory examination fees. To find out each of the factors that influence the real cost and INA-CBGs, the Kruskall Wallis and Mann-Whitney tests are conducted, while one sample test is used to determine the comparison of real costs with the rates of INA-CBGs. The results showed that patients who were included in the inclusion criteria were patients over the age of 18 years and patients routinely performed health checks at least 3 months in Sleman Yogyakarta Hospital totaling 99 patients with 330 episodes of outpatient visits in the period September-November 2017. Total patient costs DM based on the hospital perspective in the period September to November 2017 is Rp. 151,665,050.00 with 84.45% of the most fees being for the cost of drugs and medical goods. Age, Gender, Number of Complications, Duration of DM, BMI (Body Mass Index) and Antihyperglycemic Drug Types used did not differ significantly from the total real costs of diabetes mellitus. There is a negative difference between the real costs and the INA-CBG rates of Rp. -86,664,000 for a total of 99 patients with 330 episodes of outpatient visits in each code of INA CBGs namely Q-5-44-0, Z-323-0, M-3-16-0 where (p = <0 , 05) shows the significance of the difference between the real costs and the rates of INA-CBGs.
