PERBEDAAN PERUBAHAN CHILD PUGH SCORE PADA PENDERITA SIROSIS HATI YANG DITERAPI DENGAN SIMVASTATIN 20 mg DIBANDINGKAN DENGAN SIMVASTATIN 10 mg

Latar Belakang: Mortalitas dan morbiditas dari sirosis hati masih cukup tinggi di dunia. Sirosis hati dapat menyebabkan komplikasi berupa hipertensi portal dan kegagalan fungsi sintesis maupun metabolisme sel hati Skor child pugh merupakan parameter prognosis yang paling sering digunakan karena mencerminkan fungsi sintesis maupun eliminasi sel hati. Simvatatin merupakan obat yang memiliki efek anti fibrosis dan menurunkan hipertensi porta dengan meningkatkan NO sehingga hal ini menarik untuk diteliti adanya pengaruh pemberian simvastatin 10 mg dibandingkan 20 mg terhadap perubahan skor child pugh. Tujuan penelitian: Untuk mengetahui perbedaan perubahan skor Child Pugh pada penderita sirosis hati yang diberikan simvastatin 20 mg dibandingkan 10 mg selama 3 bulan. Metode: Penelitian ini merupakan penelitian eksperimental dengan rancang acak buta ganda. Subyek penelitian adalah pasien sirosis hati Child Pugh A dan B yang rutin kontrol ke klinik gastroenterohepatologi RSUP. Dr. Sardjito. Untuk mengetahui perbedaan skor child pugh sebelum dan sesudah terapi simvastatin 10 dan 20 mg dengan wilcoxon dan untuk pengetahui perbedaan selisih kedua kelompok digunakan uji mann witney. Hasil penelitian: Subyek penelitian ini terdiri dari 21 pasien pada kelompok simvatatin 10 mg dan 17 pasien pada kelompok simvastatin 20 mg. Pada kelompok simvastatin 10 mg mengalami penurunan skor Child Pugh namun tidak bermakna dari median skor Child pugh 6,0 (5,0-9,0) (95% CI= 5,84-7,063) menjadi 6,0 (5,0- 9,0) (95% CI=5,46-6,44) dengan p=0,064. Pada pasien yang mendapatkan simvastatin 20 mg tidak didapatkan perbedaan yang bermakna median skor Child Pugh 6,0 (5,0-9,0) (95% CI=5,44-6,90) menjadi 6,0 (5,0-9,0) (95% CI=5,44-6,90) dengan p=0,429. Selisih perubahan perbedaan skor Child Pugh diantara kedua kelompok bermakna dengan p=0,032. Kesimpulan : Terdapat perbedaan selisih () skor Child Pugh yang bermakna pada penderita sirosis hati yang diterapi dengan simvastatin 10 mg per hari dibandingkan dengan simvastatin 20 mg per hari selama 3 bulan.

Background: Mortality and morbidity of cirrhosis around the world is very high. The complication of hepatic cirrhosis are portal hipertension ,failure of hepatic function of syntesis and metabolism. Child Pugh score is the most often parameter to prognose patient survival because its showed function of hepatic synthesis and elimination. Simvastatin is the drug which showed effect to decrease portal hipertension by increased NO, because that its very interesting to know how the effect of ten miligram simvastatin than twenty miligram simvastatin in Child Pugh score alteration. Aim: To know differentiation of child pugh score in hepatic cirrhosis Child Pugh A and B patients who treated by simvastatin 20 mg than simvastatin 10 mg for three months. Methode: This is a double blind randomized trial. Subject of this trial are hepatic cirrhosis Child Pugh A and B patients whom visit routinly gastroenterology clinic in Sardjito Hospital. Wilcoxon is used to know the differentiation of Child Pugh score before and after treatment. Mann whitney test is used to measure change of difefferentiation between two groups. Result: subject of this trial consist of 21 patient in simvastatin 10 mg group and 17 patients in simvastatin 20 mg group. In simvastatin 10 mg group, Child Pugh score decreased but not significant which median of Child Pugh score 6,0 (5,0-9,00) (95% CI= 5,84-7,063) to 6,00 (5,00-9,00) (95% CI=5,46-6,44) p=0,064. In simvastatin 20 mg group, the differentiation of child pugh score is not significant, which median of child pugh score 6,0 (5,0-9,00) (95% CI=5,44-6,90) to 6,0 (5,00-9,00) (95% CI=5,44-6,90) p=0,429. The changes of difefferentiaton between two groups is significant p=0,032. Conclution: There was a significant difference in child pugh scores in patient with liver chirrhosis treated with simvastatin 10 mg daily compared to simvastatin 20 mg daily for 3 months.
