DISPARITAS TARIF INA-CBGS DAN TARIF RUMAH SAKIT PASIEN BPJS RAWAT INAP DI RSUD KOLONODALE, KAB MOROWALI UTARA

Latar Belakang : Rumah sakit sebagai fasilitas rujukan tingkat lanjut memiliki peran penting dalam implementasi program Jaminan Kesehatan Nasional (JKN). Sistem Jaminan Kesehatan Nasional (SJSN) yang di selenggarakan oleh Badan Penyelenggara Jaminan Kesehatan (BPJS) membawa perubahan dalam sistem pembayaran pelayanan kesehatan pada fasilitas rujukan tingkat lanjut atau Rumah Sakit dimana Tarif INA-CBGs yang ditetapkan pemerintah dengan mekanisme pembayaran secara prospektif, menyebabkan terjadinya disparitas tarif antara tarif INA-CBGs dan tarif rumah sakit (fee for service) yang ditetapkan sesuai dengan Peraturan Bupati Morowali Utara No 13 tahun 2015 yang berlaku di RSUD Kolonodale. Data klaim rawat inap Januari -September 2017 RSUD Kolonodale menunjukan disparitas tarif sebesar Rp.-1.085.153.570. Oleh karena itu perlu dilakukan analisa biaya yang tepat dengan menghitung unit cost sehingga dapat memberikan informasi yang akurat atas biaya yang dibutuhkan pada penanganan sebuah kasus. Tujuan : Mengidentifikasi kasus yang menunjukan kesenjangan tarif antara tarif INA CBGs dan tarif rumah sakit dengan kriteria kasus high volume, high cost, high risk yang menjalani rawat inap di RSUD Kolonodale yang di jamin oleh BPJS pada bulan Oktober 2017-Desember 2017 dan menghitung biaya satuan atau unit cost dari kasus tersebut. Metode : Penelitian ini merupakan penelitian deskriptif dengan melaksanakan studi dokumen yang terkait dengan penelitian ini dan melakukan observasi pada aktivitas prosedur persalinan sectio caecaria terkait dengan biaya yang timbul akibat aktivitas tersebut. Hasil Penelitian : Hasil identifikasi kasus diperoleh bahwa kasus sectio caecaria tanpa penyulit memenuhi kriteria sampel yang diteliti.Disparitas tarif INA-CBGs dan tarif rumah kasus sectio caecaria Oktober-Desember 2017 sebesar Rp.- 110,530,344. Hasil perhitungan unit cost pada masing-masing kelas perawatan diperoleh Kelas VIP sebesar RP. 7,262,468, Kelas I Rp. 6,365,943 dan Kelas III Rp. 6,212,279. Kesimpulan : Ada selisih antara Tarif INA-CBGs dan Unit Cost dimana Tarif INA CBGs lebih rendah dari unit cost untuk pelayanan bedah Sectio Ceacaria tanpa penyulit pada kelas perawatan VIP, Kelas I maupun Kelas III yang artinya terdapat potensi kerugian bagi rumah sakit.

Background: Hospital as an advanced referral facility has an important role in the implementation of the National Health Insurance program (Jaminan Kesehatan Nasional or JKN). The National Health Insurance System (Sistem Jaminan Kesehatan Nasional or SJSN) held by the Health Insurance Organizing Agency (Badan Penyelenggara Jaminan Kesehatan or BPJS) brings changes in the payment system for health services at advanced referral facilities or Hospitals where the INA- CBGs Tariffs set by the government with a prospective payment mechanism cause disparity between the INA-CBGs tariff and the hospital tariff (fee for service) determined in accordance with the North Morowali District Head Regulation No. 13 of 2015 which applies in Kolonodale Hospital. Inpatient claim data from January to September 2017 Kolonodale General Hospital shows disparity in tariff of IDR. 1,085,153,570. Therefore, it is necessary to do an appropriate cost analysis by calculating the unit cost to provide accurate information on the costs required in handling a case. Objective: To identify cases denoting cost disparities between INA-CBG and hospital rate using with criteria including high volume, high cost, high risk which underwent inpatient care in Kolonodale with BPJS insurance between October - December 2017 and calculating unit cost of those cases. Method: This study is a descriptive study by doing document study related to the topic and observing sectio caesarea delivery procedure related to the cost needed on each case. Results: The results of the case identification found that the tariff difference of INA- CBGs and the hospital rate of sectio caesaria cases from October to December 2017 was IDR. 110,530,344. Results of the unit cost calculation for each class of care are IDR. 7,262,468 for VIP, IDR. 6,365,943 for Class I and IDR. 6,212,279 for Class III. Conclusion: There are discrepancies between the INA-CBGs Tariff and Unit Cost where the INA CBGs Tariff is lower than the unit cost for uncomplicated sectio caesaria surgery service for VIP, Class I and Class III treatment classes which means there is a potential loss for the hospital.
