INTENSI KEMITRAAN RUMAH SAKIT SWASTA DENGAN BPJS KESEHATAN SEBAGAI PROVIDER PROGRAM JAMINAN KESEHATAN NASIONAL DI KOTA DENPASAR

Latar Belakang: Perkembangan Rumah Sakit Swasta pasca diberlakukannya program JKN mengalami peningkatan yang cukup signifikan. Walaupun Rumah Sakit Swasta tidak diwajibkan untuk bekerjasama sebagai provider program JKN, akan tetapi kontribusinya perlu untuk dipertimbangkan. Hal ini tentu terkait dengan jumlah peserta yang akan terus bertambah dan membutuhkan lebih banyak fasilitas pelayanan kesehatan. Di Provinsi Bali ada tiga Rumah Sakit Swasta yang menghentikan kerja sama terhitung mulai 31 Desember 2016, dua diantaranya merupakan rumah sakit yang terletak di Kota Denpasar. Dengan tingginya pertumbuhan penduduk serta pemanfaatan layanan rawat inap Rumah Sakit Swasta yang semakin meningkat, maka perlu untuk diketahui gambaran intensi kemitraan Rumah Sakit Swasta dengan BPJS Kesehatan dalam memperkuat komitmen penyediaaan layanan kesehatan rujukan kepada peserta program JKN di Kota Denpasar. Metode Penelitian: Penelitian ini merupakan penelitian kualitatif dengan rancangan studi kasus. Penelitian ini dilakukan pada 2 Rumah Sakit Swasta yang sedang bekerja sama dan 2 Rumah Sakit Swasta yang tidak bekerja sama dengan BPJS Kesehatan di wilayah Kota Denpasar. Metode pengambilan informan yang digunakan adalah purposive sampling dengan total 8 orang informan. Proses pengumpulan data menggunakan teknik wawancara mendalam dan dipandu dengan pedoman wawancara. Hasil: Dari sisi tarif, besaran paket INA-s yang ditetapkan selama ini cukup jauh berbeda dengan perhitungan unit cost dari rumah sakit. Sistem rujukan yang selama ini diberlakukan dapat menekan biaya pelayanan dan sangat tepat dilakukan dalam program JKN. Dari sisi sistem kontrak, ada komitmen rumah sakit untuk melakukan kerja sama karena takut kehilangan pasien serta pelaksanaan kredensialing sudah sesuai dengan regulasi. Dalam dinamika regulasi pada Permenkes tentang standar tarif pelayanan, perubahan yang terjadi begitu cepat menyebabkan beberapa rumah sakit mengundurkan diri/tidak melanjutkan kerja sama dengan BPJS Kesehatan. Kesimpulan: Rumah Sakit Swasta yang sudah bekerja sama maupun tidak bekerja sama menunjukan intensi kemitraan dengan BPJS Kesehatan. Faktor yang menjadi pertimbangan adalah sikap terhadap tarif dan kontrol perilaku terhadap regulasi yang negatif. Sementara sikap terhadap sistem rujukan dan sistem kontrak yang positif tidak menjadi pertimbangan dalam melakukan kemitraan dengan BPJS Kesehatan.

Background: The development of Private Hospital after the enactment of the JKN program has increased significantly. Although Private Hospital is not required to cooperate as a provider of JKN program, its contribution needs to be considered. This is certainly related to the number of participants who will continue to grow and require more healthcare facilities. In Bali, there are three Private Hospitals that terminate partnerships as of 31 December 2016, two of which are hospitals located in Denpasar. With the high population growth and the increasing utilization of inpatient services of Private Hospital, it is necessary to know the description of partnership intention of Private Hospital with BPJS Kesehatan in strengthening the commitment of the provision of referral health services to participants of JKN program in Denpasar. Methods: This research is a qualitative research with case study approach. This study was conducted on 2 Private Hospitals that were in partnership and 2 Private Hospitals that were not in partnership with BPJS Kesehatan in Denpasar. The method of taking informants used is purposive sampling with a total of 8 people informant. The process of collecting data uses in-depth interview techniques and guided by interview guidelines. Results: In terms of tariffs, the amount of INA-CBG's packages established so far is quite different from unit cost calculations from hospitals. Referral system that has been applied can reduce the cost of service and is very appropriate in the JKN program. In terms of the contract system, there is a commitment of the hospital to cooperate because of the fear of losing patients and the implementation of credence is in accordance with the regulations. In the regulatory dynamics at Permenkes on tariff standards, the changes that occurred so quickly caused some hospitals to resign/discontinue their partnership with BPJS Kesehatan. Conclusion: Private Hospitals that have cooperated or not cooperated show the intention of partnership with BPJS Kesehatan. Factors to be considered are negative attitudes toward tariff and negative behavior controls toward the regulation. While the attitude towards the referral system and attitude towards the contract system are positive, not a consideration in its intention to conduct partnership with BPJS Kesehatan.
