PENGARUH PERAWATAN PALIATIF TERHADAP KUALITAS HIDUP PADA PASIEN KANKER DI KOMUNITAS: LITERATURE REVIEW

Latar Belakang: Perawatan paliatif bertujuan untuk meningkatkan kualitas hidup pasien dan keluarga yang menghadapi penyakit yang mengancam jiwa. Salah satu penyakit yang membutuhkan perawatan paliatif adalah kanker. Penyakit kanker dapat menyebabkan masalah fisik, psikologis, sosial, dan spiritual bagi penderitanya, sehingga kualitas hidup pasien akan menurun. Perawatan paliatif dibutuhkan disemua level perawatan termasuk komunitas namun sebagian besar penelitian mengenai perawatan paliatif berada di rumah sakit. Tujuan Penelitian: Mengetahui pengaruh pemberian perawatan paliatif terhadap kualitas hidup pasien kanker di komunitas. Metode: Penelitian ini merupakan penelitian literature review. Database yang digunakan ScienceDirect, PubMed, MEDLINE, dan Cochrane Library. Kriteria inklusinya, yaitu: literatur berbahasa Inggris, tahun terbit 2009 hingga 2018, populasi penelitian literatur adalah pasien kanker dewasa, tersedia dalam full-text. Kriteria eksklusinya: sumber buku, seminar, review, dan konferensi. Hasil: Berdasarkan hasil penelusuran literatur didapatkan 3 artikel. Pengaruh perawatan paliatif di komunitas terhadap kualitas hidup pasien kanker terdiri dari 4 domain. Domain fisik berupa menurunnya tingkat nyeri, kelelahan, mual, gangguan pencernaan, insomnia, gangguan pernafasan, penyembuhan luka, dan komplikasi lain akibat kanker. Domain psikologis: mengurangi tingkat depresi pasien, teratasinya ketakutan, stigma sosial, dan sikap terhadap pasien kanker. Domain sosial: pengurangan biaya perawatan untuk hospitalisasi. Domain spiritual: keluarga pasien yang meninggal karena kanker menjadi lebih mempercayai tuhan, dukungan keluarga dan teman, sikap positif, dan percaya pada takdir Kesimpulan: Pemberian palliative care yang berada di komunitas memiliki pengaruh pada kualitas hidup pasien kanker dengan meringankan gejala fsik, psikologis, sosial, dan meningkatkan spiritual pasien

Background: Palliative care aims to improve the quality of life (QoL) of patients and families who face life-threatening illnesses, such as cancer. Cancer can cause physical, psychological, social, and spiritual problems for the sufferer, so the QoL of the patient will decrease. Palliative care is required at all levels of care including the community but most studies on palliative care are in the hospital. Objective: To determine the effect of palliative care on the QoL of cancer patients in the community. Method: This research is a literature review research. The database which used were ScienceDirect, PubMed, MEDLINE, and Cochrane Library. The inclusion criteria: English literature, from 2009 to 2018, the population of literature research is adult cancer patients, available in full-text. Exclusion criteria: sources of books, seminars, reviews, and conferences. Result: Based on the literature search results obtained 3 articles. The effect of palliative care in the community on the QoL of cancer patients consists of 4 domains. Physical domains include decreased levels of pain, fatigue, nausea, indigestion, insomnia, respiratory disorders, and wound healing. Psychological domains: reduce depression level, fear, social stigma, and attitudes toward cancer patients. Social domain: reduction of maintenance costs for hospitalization. Spiritual Domain: The families of patients who die from cancer become more trusting in gods, family and friends support, positive attitude, and believe in destiny Conclusion: Provision of palliative care residing in the community has an influence on the QoL of cancer patients by relieving physic, psychological, social, and spiritual symptoms of the patient.
