PERBEDAAN EFEKTIVITAS FIKSASI MEMBRAN ANTARA TEKNIK PERIOSTEL VERTICAL MATTRESS DAN SIMPLE SLING SUTURE PADA PERAWATAN POKET INFRABONI (KAJIAN PADA PROBING DEPTH, RELATIVE ATTACHMENT LOSS DAN TINGGI TULANG ALVEOLAR)

Guide Tissue Regeneration (GTR) merupakan perawatan poket infraboni bertujuan untuk meregenerasi jaringan lunak maupun jaringan keras. Membran digunakan sebagai penghalang dan mencegah migrasinya sel-sel pembentuk jaringan epitel ke arah apikal. Fiksasi membran merupakan salah satu prosedur dalam perawatan GTR karena membran yang sifatnya teresorpsi kurang stabil. Teknik fiksasi membran simple sling suture memiliki 1 penjangkaran yang terletak pada koronal flap sedangkan teknik fiksasi membran periosteal vertical mattress suture memiliki 2 penjangkaran pada periosteum sehingga lebih stabil. Tujuan penelitian ini adalah mempelajari perbedaan efektivitas fiksasi membran antara teknik periosteal vertical mattress suture dan simple sling suture yang ditinjau dari probing depth, relative attachment loss, dan tinggi tulang alveolar pada terapi poket infraboni. Sampel penelitian dibagi menjadi 2 kelompok. Kelompok pertama yaitu Open flap debridement (OFD) ditambah aplikasi DFDBA dan membran dengan fiksasi membran simple sling suture sedangkan kelompok kedua OFD ditambah aplikasi DFDBA dan membran dengan fiksasi membran periosteal vertical mattress suture yang diperiksa pada bulan 0, 1, dan 3. Hasil penelitian menunjukkan bahwa terdapat perbedaan signifikan pada probing depth dan relative attachment loss (kecuali pada hari ke-30 ke hari ke-90), dan tidak terdapat perbedaan signifikan pada tinggi tulang alveolar mulai baseline ke hari ke-90, antara kelompok fiksasi membran simple sling suture dan fiksasi membran periosteal vertical mattress suture pada terapi bedah flap. Kesimpulan dari penelitian ini adalah fiksasi membran pada perawatan poket infraboni dengan teknik periosteal vertical mattress suture lebih efektif menurunkan probing depth dan relative attachment loss, serta tidak ada perbedaan dalam menaikkan tinggi tulang alveolar dibanding fiksasi membran dengan teknik simple sling suture

Guide Tissue Regeneration (GTR) is an infrabony pocket treatment aimed to regenerating soft tissue and hard tissue. Membranes are used as a barrier and prevent the migration of epithelial tissue-forming cells to apical direction. Membrane fixation is one of the procedures in GTR treatment because the resorption membrane is less stable. A simple sling suture membrane fixation technique has one anchorage located on the coronal flap while the periosteal membrane perientaleal membrane fixation technique has 2 anchorages on the periosteum so that it is more stable. The purpose of this research is to study the effectiveness of membrane fixation between periosteal vertical mattress suture and simple sling suture technique from probing depth, relative attachment loss, and alveolar bone height in infrabony pocket therapy. The study sample was divided into 2 groups. The first group was Open flap debridement (OFD) plus DFDBA and membrane applications with simple sling suture membrane fixation while the second group of OFD plus DFDBA and membrane applications with periosteal vertical mattress suture membrane fixation were examined at months 0, 1, and 3. The results showed that there are significant differences in probing depth and relative attachment loss (except on day 30 to day 90), and there is no significant difference in alveolar bone height from baseline to day 90, between the simple sling suture membrane fixation group and periosteal vertical mattress suture membrane fixation on flap surgical therapy. The conclusion of this research is the membrane fixation on the treatment of infraboni pocket with the technique of periosteal vertical mattress suture more effectively decrease probing depth and relative attachment loss, and there is no difference in raising alveolar bone height compared to membrane fixation with simple sling suture
