Penelitian merupakan sektor yang cukup penting bagi studi mengenai kesehatan masyarakat. Dalam suatu survei penelitian yang melibatkan peran masyarakat, konsistensi dan validitas data merupakan faktor yang sangat penting agar hasil penelitian dapat lebih meyakinkan. Di sisi lain, perkembangan teknologi informasi seperti internet sudah pada tahap dimana masyarakat tidak bisa dipisahkan dari teknologi informasi. Berangkat dari masalah tersebut, pihak FKKMK UGM memiliki inisiatif untuk membuat suatu platform yang diberi nama ABCD Online Data Collection yang memanfaatkan teknologi informasi untuk membantu peneliti yang meneliti mengenai kesehatan masyarakat. Dengan platform tersebut maka peneliti dapat terbantu dengan data yang lebih valid, karena pengolahan data dilakukan oleh komputer. Selain itu, platform tersebut juga dikhususkan untuk peneliti di bidang kesehatan masyarakat sehingga memiliki fitur-fitur yang dibutuhkan oleh peneliti di bidang kesehatan masyarakat. Dalam mengembangkan platform ABCD Online Data Collection, dibutuhkan basis data yang dapat menyimpan data hasil pengumpulan survei yang telah diisi secara online oleh masyarakat atau responden. Basis data yang dibuat harus disesuaikan dengan fitur yang terdapat pada aplikasi ABCD Online Data Collection, tidak memuat redundansi data, dan dapat melakukan operasi CRUD (Create Read Update Delete). Untuk membuat basis data dengan kriteria tersebut, perancangan basis data pada penelitian ini menggunakan metode normalisasi data sampai bentuk 3NF. Penelitian ini menghasilkan basis data yang diimplementasikan pada aplikasi sistem informasi ABCD Online Data Collection yang dibuat dalam DBMS MySQL.

Research is an important sector for public health's studies. In a research that involving the role of society, consistency and validity of the data are very important factor for the more properly result of the research. On the other hand, the development of information technology such as internet nowadays has reached the stage where the society's life can't be separated from the use of information technology. Based on those problems, researcher from FKKMK UGM had an idea to develop a platform named ABCD Online Data Collection that using the information technology to help the researchers who are researching about public health. With this platform, researcher will be helped with data that have more validity, because the data processing is done by computer. Furthermore, this platform is also specialized for the public health's researcher so it has features that are needed by researchers in the public health's field. On developing the ABCD Online Data Collection platform, it needs the database that can store the data collection from survey's results that have been filled online by the society or respondents. The database should be adapted with the features contained in the ABCD Online Data Collection's application, does not contain data redundancy, and can perform CRUD (Create, Read, Update, Delete) operations. To create a database with these criteria, the database design in this study will using the method of data normalization to 3NF form. This research produces a database that is implemented on the ABCD Online Data Collection information system application and created in the MySQL DBMS.
