ANALISIS FAKTOR PENYEBAB DAN UPAYA PENYELESAIAN MASALAH DUPLIKASI NOMOR REKAM MEDIS DI UPT PUSKESMAS PURWOREJO

Latar Belakang: Rekam medis yang baik adalah rekam medis yang memiliki data berkesinambungan, sejak awal hingga akhir perawatan. Kesinambungan data rekam medis merupakan satu hal yang mutlak dipenuhi dalam menjaga nilai rekam medis yang baik untuk mendukung kesehatan yang maksimal (Huffman,1994). Di UPT Puskesmas Purworejo, ditemukan kejadian duplikasi nomor rekam medis yang menyebabkan ketidaksinambungan informasi perawatan pasien. Tujuan: Mengetahui analisis faktor penyebab dan upaya penyelesaian masalah duplikasi nomor rekam medis di UPT Puskesmas Purworejo. Metode: Penelitian ini menggunakan metode deskriptif dengan pendekatan kualitatif dan rancangan penelitian studi kasus. Subjek penelitian adalah petugas pendaftaran dan filing. Objek penelitian adalah register kunjungan pasien bulan Februari 2018, Indeks Utama Pasien dan berkas rekam medis pasien yang mengalami duplikasi. Hasil: Sistem penomoran yang digunakan di Puskesmas Purworejo ialah Unit Numbering System. Persentase kejadian duplikasi kategori 1 (satu pasien memiliki lebih dari satu nomor rekam medis) sebesar 9,32% dan persentase duplikasi kategori 2 (satu berkas rekam medis dimiliki lebih dari satu pasien) sebesar 4,29%. Faktor penyebab kejadian duplikasi meliputi faktor man, material, machine dan method. Alternatif solusi meliputi peningkatkan pendidikan petugas rekam medis, pengadaan kegiatan pelatihan, edukasi pasien mengenai pentingnya KIB, pemutakhiran IUP, pengadaan bank nomor rekam medis, pengadaan SOP penamaan pasien, SOP penomoran pasien dan SOP penananganan kejadian duplikasi nomor rekam medis, serta peningkatkan ketelitian petugas dalam proses identifikasi pasien. Kesimpulan: Pengadaan bank nomor rekam medis dan pembuatan SOP dipilih sebagai solusi terbaik dalam penyelesaian duplikasi nomor rekam medis di Puskesmas Purworejo. Background: A good medical record is a medical record that has continuous data, from the beginning to the end of the treatment. The continuity of medical record data is one thing that is absolutely fulfilled in maintaining a good medical record value to support maximum health (Huffman, 1994). In UPT Puskesmas Purworejo, there was an incident of duplication of medical record number that caused the discontinuation of patient care information. Objective: To know the factor analysis of cause and effort to solve the problem of medical record number duplication in UPT Puskesmas Purworejo. Methods: This research used descriptive method with qualitative approach and case study research design. The subjects of the study were the registration and filing officer. The object of the study was to register patient visits in February 2018, Master Patient Index and medical records of duplicated patients. Results: The numbering system used is Unit Numbering System. Percentage of incidence of category 1 duplication (one patient has more than one medical record number) of 9.32% and the percentage of duplication category 2 (one medical records file owned by more than one patient) of 4.29%. Factors that cause duplication include man, material, machine and method. Alternative solutions include improving the education of medical record officers, provision of training activities, patient education on the importance of KIB, updating of IUP, procurement of bank number of medical record, procurement of patient naming SOP, patient numbering SOP, SOP handling of medical record number duplication events, and increased accuracy of officers in the process of identifying patients. Conclusion: The procurement of bank number of medical record and the making of SOP was chosen as the best solution in completion of duplication of medical record number at UPT Puskesmas Purworejo.
