PENGARUH PIJAT REFLEKSI KAKI TERHADAP PENURUNAN CANCER RELATED FATIGUE PADA PASIEN KANKER

Pijat refleksi kaki termasuk Complementary Alternative Medicine (CAM) untuk mengurangi CRF. Praktek pijat refleksi kaki banyak dilakukan di masyarakat, namun di Indonesia belum terdapat penelitian pijat refleksi kaki terhadap CRF. Penelitian bertujuan mengetahui pengaruh pijat refleksi kaki terhadap penurunan CRF pada pasien kanker. Metode: Penelitian dilakukan Januari sampai Februari 2018 di salah satu rumah sakit rujukan di Jawa Tengah. Penelitian intervensi dengan desain pre-test and post-test with control group, melibatkan 67 responden ditempatkan melalui block randomization (33 kelompok intervensi dan 34 kelompok kontrol). Allocation conceallment menggunakan amplop. Pijat refleksi kaki diberikan oleh teknisi tersertifikasi, 1 x sehari (30 menit /sesi) selama 2 hari, kelompok kontrol menerima perawatan standar. Pengukuran outcome menggunakan Visual Analouge Scale Fatigue (VAS-F). Uji hipotesis menggunakan independent sample t test. Uji besar efek menggunakan Cohen d. Hasil: Karakteristik responden kedua kelompok homogen (p>0,05). Skor CRF kelompok intervensi dan kontrol pada pengukuran awal 5,1 ± 2,3 dan 5,3 ± 2,4; pengukuran akhir 2,2 ± 1,8 dan 4,8 ± 2,6. Selisih CRF kelompok intervensi 2,9 ± 2,2 dan kontrol 0,5 ± 1,5 dengan p=0,001 (p<0,05), CI [(-3,3) - (-1,4)], t = -5,2 dan besar efek cohen d = 1,27. Outcome dikotomus CRF dibedakan penurunan bermakna vs tidak bermakna yaitu kelompok intervensi 25 (75,8%) vs 8 (24,2%) dan kontrol 9 (26,5%) vs 25 (73,5%). Kesimpulan: pijat refleksi kaki berpengaruh besar terhadap penurunan CRF pada pasien kanker.
Background: Cancer Related Fatigue (CRF) is a symptom of cancer patients with high prevalence and impact on quality of life. Foot reflexology massage includes Complementary Alternative Medicine (CAM) to reduce CRF. The practice of foot reflexology massage is widely practiced in the community, but in Indonesia there have been no foot reflexology massage research on CRF. The objective of this study is to know the effect of foot reflexology massage on CRF reduction in cancer patients. Method: The research was conducted from January to February 2018 at one of the referral hospitals in Central Java. Intervention study pre-test and post-test with control group, involving 67 respondents divided by block randomization (33 intervention groups and 34 control groups). Allocation concealment is using envelope. Foot reflexology massage is given by certified technicians, once a day (30 minutes / session) for 2 days, while the control group receives standard care. Outcome measurements using Visual Analogue Scale - Fatigue (VAS-F). Hypothesis test using independent sample t test. Effect size analysis using Cohen d. Result: the characteristic respondent of both groups were homogeneous (p> 0.05). CRF score of the intervention and control group at baseline were 5,1 ± 2,3 dan 5,3 ± 2,4; post measurement were 2,2 ± 1,8 dan 4,8 ± 2,6. The CRF difference of the intervention group is 2.9 ± 2.2 and the control is 0.5 ± 1.5 with the value of p = 0.001 (p <0.05), CI [(-3.3) - (-1.4)], t = -5.2 and the effect of cohen d = 1.27. The outcomes of CRF dichotomous were differentiated meaningful and meaningless decline, i.e. intervention group 25 (75.8%) vs 8 (24.2%) and control 9 (26.5%) vs 25 (73.5%). Conclusion: foot reflexology massage affects the decrease of CRF in cancer patients.
