PENGARUH APLIKASI TOPIKAL EKSTRAK KULIT BUAH MANGGIS TERHADAP EKSPRESI VASCULAR ENDOTHELIAL GROWTH FACTOR (VEGF) PADA ALVEOLITIS PASCA PENCABUTAN GIGI (studi eksperimental tikus sebagai model)

Latar belakang. Alveolitis merupakan komplikasi pencabutan gigi permanen yang mengindikasikan inflamasi yang berlebihan pada tulang alveolar maksila atau mandibula. Ekstrak kulit buah manggis diperkirakan mampu mencegah produk inflamasi yang berlebihan dengan mempercepat apoptosis dan mempertahankan kondisi sel endotelial vaskular. Vascular Endothelial Growth Factor (VEGF) merupakan faktor angiogenik utama dalam mitogen spesifik sel endotelial, kemotaksis, dan menginduksi permeabilitas vaskular. Penelitian ini menganalisis ekstrak kulit buah manggis secara topikal terhadap ekspresi VEGF pada model alveolitis pasca pencabutan gigi tikus. Metode. Dua puluh empat ekor tikus wistar jantan, umur 10 minggu, berat badan 200�250 gr digunakan dalam penelitian ini. Tikus dicabut gigi insisivus kanan maksila dan dibagi 4 kelompok, yaitu kelompok 1 model normal, kelompok 2 model alveolitis yang diinduksi adrenalin perbandingan 1:1000 selama 15 menit dan diobservasi klinis setelah 48 jam ditandai hiperemia subepitelial perialveolar hingga terbentuknya nekrotik jaringan. Kelompok 3 model preventif setelah induksi adrenalin diaplikasikan ekstrak kulit buah manggis konsentrasi etanol 40%, sedangkan kelompok 4 aplikasi ekstrak pada model alveolitis. Pemeriksaan ekspresi VEGF dilakukan dengan immunohistokimia pada dekapitasi hari ke-3, ke7, ke-14. Hasil. Analisis anova dua jalur dilanjutkan dengan uji post hoc ditemukan perbedaan ekspresi VEGF yang signifikan (p<0,05) hari ke-7 pada aplikasi topikal ekstrak kulit buah manggis konsentrasi etanol 40% pada model alveolitis (penurunan sebesar 4,1%) dan model preventif (penurunan sebesar 6,9%) dibandingkan tanpa ekstrak pada model alveolitis (penurunan sebesar 32,9%) hingga hari ke-14 pasca pencabutan gigi. Kesimpulan. Aplikasi topikal kulit buah manggis meningkatkan ekspresi VEGF pada model alveolitis pasca pencabutan gigi insisivus hewan tikus.

Background. Alveolitis is a permanent tooth extraction-induced complication, which indicates excessive inflammation of the maxillary or mandibular alveolar bone. The mangosteen peel extract is thought to prevent excessive inflammatory products by accelerating apoptosis and maintaining vascular endothelial cell conditions. The Vascular Endothelial Growth Factor (VEGF) compound is a major angiogenic factor in endothelial cell-specific mitogen, chemotaxis, and induces vascular permeability. This study analyzed the effects of topically-applied mangosteen peel extract on VEGF expression on rat tooth extraction-post alveolitis model Method. Twenty four male Wistar rats, aged 10 weeks, 200-250 grams in body weight were involved, in which the rat�s right maxillary incisor was extracted and the rats were divided into 4 groups, comprising Group 1, the normal model, Group 2, the adrenaline- induced alveolitis model by the ratio of 1: 1000 within 15 minutes and received clinical observation after 48 hours marked with perialveolar subepithelial hyperemia leading to tissue necrotic formation. Group 3, the preventive model, following adrenaline induction received an application of mangosteen peel extract with 40% ethanol concentration, and Group 4, alveolitis receiving the extract application. Examination of VEGF expression was performed with immunohistochemistry on decapitation day 3,7 and 14. Results. A two-way ANOVA analysis revealed that the topical application of mangosteen peel extract with 40% ethanol concentration resulted in was a significant difference of VEGF expression on day 7 with p <0.05 on the alveolitis mode (decreasing by 4.1%) and the preventive model (decreasing by 6. 9%) compared with the one without extract application on the alveolitis model (decreasing by 32.9%) after the rat�s incisor extraction. Conclusion. Topical application of mangosteen peel extract increased VEGF expression in rat�s incisor extraction-post alveolitis model.
