ANALISIS KELENGKAPAN PENGISIAN RESUME MEDIS DALAM PEMENUHAN ELEMEN PENILAIAN STANDAR ARK 4.2 AKREDITASI SNARS DI RSU PKU MUHAMMADIYAH NANGGULAN KULONPROGO

Latar Belakang : Ketidaklengkapan resume medis berdampak pada ketidaksinambungan informasi kesehatan maka akan berpengaruh pada mutu pelayanan kesehatan sedangkan RSU PKU Muhammadiyah Nanggulan belum pernah dilakukan akreditasi dan sedang persiapan akreditasi. Dalam akreditasi SNARS terdapat standar ARK 4.2 yang berkaitan dengan penilaian resume medis. Tujuan : Menganalisis kelengkapan pengisian resume medis terkait pemenuhan elemen penilaian Standar ARK 4.2 dalam akreditasi SNARS di RSU PKU Muhammadiyah Nanggulan. Metode : Jenis penelitian adalah penelitian deskriptif kualitatif dan rancangan penelitian studi kasus. Subjek dalam penelitian ini adalah petugas rekam medis, dokter spesialis/DPJP dan perawat bangsal. Objek dalam penelitian ini yaitu lembar resume medis pasien rawat inap. Teknik pengumpulan data dengan wawancara, observasi, dan studi dokumentasi. Uji keabsahan data menggunakan triangulasi sumber. Hasil : Lembar resume medis diisi oleh DPJP setelah visit dan pasien dinyatakan boleh pulang. Tetapi di RSU PKU Muhammadiyah Nanggulan, perawat bangsal juga membantu mengisi lembar resume medis rawat inap. Pemenuhan EP 1 terpenuhi sebagian, pemenuhan EP 2 terpenuhi lengkap, pemenuhan EP 3 terpenuhi sebagian, pemenuhan EP 4 terpenuhi sebagian, pemenuhan EP 5 terpenuhi sebagian, pemenuhan EP 6 terpenuhi sebagian. Upaya petugas rekam medis dalam mengatasi ketidaklengkapan dengan mengembalikan berkas kepada DPJP pasien di bangsal. Kesimpulan : Pelaksanaan pengisian lembar resume medis belum sesuai dengan peraturan yang ada karena di RSU PKU Muhammadiyah Nanggulan belum adanya SPO terkait pengisian lembar resume medis sehingga dalam pemenuhan elemen penilaian Standar ARK 4.2 Akreditasi SNARS masih terpenuhi sebagian.

Background: The incompleteness of the discharge summary affects the discontinuity of health information it will be affect the quality of health service while RSU PKU Muhammadiyah Nanggulan has never been done accreditation before and still preparing for the accreditation. In the SNARS accreditation there are ARK 4.2 Standards relating to discharge summary assessment. Purpose: To analysis completeness of discharge summary related to the fulfillment of assessment element ARK 4.2 Standard in SNARS Accreditation in RSU PKU Muhammadiyah Nanggulan. Methods: The type of reseacrh is qualitative descriptive research and case study research design. Subjects in this study were medical record officers, the medical specialist and nurse. The object in this study is inpatients discharge summary sheets. Technique of collecting data by interview, observation, and study of documentation. Test data validity using source triangulation. Result: Discharge summary sheet filled by the medical specialist after visit was done and patient can be discharge. But in RSU PKU Muhammadiyah Nanggulan, the ward nurse also helped fill out the inpatient discharge summary sheet. The fulfillment of EP 1 is partially fulfilled, EP 2 is fully fulfilled, EP 3 is partially fulfilled, EP 4 is partially fulfilled, EP 5 is partially fulfilled, EP 6 is partially fulfilled. Medical record officer's efforts to overcome incompleteness by returning the documents to the medical specialist in the ward. Conclusion: The implementation of filling out the discharge summary is not in accordance with the existing regulations because in RSU PKU Muhammadiyah Nanggulan has not been Standard Operating Procedure(SOP) related to the filling discharge summary so that in fulfillment of assessment element ARK 4.2 Standard in SNARS accreditation still partially fulfilled.
