Peran Apoteker Di Dalam Upaya Mencegah Medication Error Di Dalam Pelayanan Resep Racikan Di Puskesmas Jetis Kota Yogyakarta

Resep racikan adalah resep yang dalam pelayanannya harus diracik terlebih dahulu. Bentuk sediaan obat yang berubah dapat mempengaruhi stabilitas dari obat. Hal ini merupakan salah satu dari faktor pemicu medication error. Metode penelitian yang digunakan adalah metode deskriptif non analitik yang bersifat non eksperimental dengan pengambilan data secara retrospektif dari resep racikan di Puskesmas Jetis Kota Yogyakarta. Tujuannya untuk membuat gambaran atau deskripsi tentang profil resep racikan di Puskesmas Jetis, kejadian medication error, dan peran apoteker secara objektif dan tanpa ada manipulasi (intervensi) dari peneliti menggunakan data yang telah ada. Kesalahan resep racikan dideteksi dengan pengamatan langsung resep racikan, membandingkan dosis dan frekuensi tercantum dalam resep racikan dengan perhitungan dosis menurut literatur, serta melihat ada tidaknya interaksi obat dengan aplikasi Medscape dan literatur. Selanjutnya dilakukan wawancara dengan apoteker di Puskesmas Jetis untuk mengumpulkan informasi yang tidak tercantum di dalam resep racikan. Berdasarkan hasil penelitian diketahui bahwa medication error telah terjadi pada pelayanan resep racikan di Puskesmas Jetis Kota Yogyakarta. Sebanyak 95,50% resep mengalami potensi medication error yang disebabkan oleh beberapa faktor antara lain tulisan tidak terbaca, singkatan tidak baku, ketidaklengkapan resep, ketidaksesuaian dosis, frekuensi, dan adanya interaksi obat. Apoteker di Puskesmas Jetis Kota Yogyakarta berperan secara langsung dalam pencegahan medication error pada resep racikan. Salah satunya dengan melakukan koreksi resep, meliputi koreksi dosis, komposisi obat, dan melakukan pengecekan interaksi obat. Selain itu, apoteker di Puskesmas Jetis juga melakukan pemberian informasi obat kepada pasien pada saat penyerahan obat dilakukan. Bentuk preventif lainnya adalah evaluasi yang dilakukan secara berkala oleh apoteker dan tenaga kesehatan di Puskesmas Jetis Kota Yogyakarta.

Prescription recipe is a recipe that in its service must be formulated first. The changing form of the dosage form may affect the stability, speed of degradation, and speed of dissolution of the drug. This is one of the trigger factors of medication error. The research method used is non analytical non analytical method that is non experimental with retrospective retrieval data from prescription recipe at Puskesmas Jetis Kota Yogyakarta. The goal is to create an overview or description of prescription concentration profiles at Jetis Health Center, medication error events, and the role of pharmacist objectively and without any manipulation (intervention) from the researchers using existing data. The prescribed concoction error is detected by direct observation of the prescribed recipe, comparing the dosage and frequency listed in the prescribed prescription by dose calculation according to the literature, as well as looking for drug interactions with Medscape and literature applications. Furthermore, interviews were conducted with pharmacists at Jetis Health Center to collect information not listed in the prescribed recipe. Based on the result of the research, it is known that medication error has occurred at the service of prescription concoction at Jetis Health Center of Yogyakarta City. A total of 95.50% of recipes experienced potential medication errors caused by several factors including prescription uncertainty, incomplete prescriptions, dose discrepancies, frequency, and drug interactions. The pharmacist at Jetis Health Center of Yogyakarta City has a direct role in preventing medication error on prescription recipe. One of them is by prescribing corrections, including dose correction, drug composition, and checking drug interactions. In addition, pharmacists at Puskesmas Jetis also provide drug information to patients at the time of drug delivery. Another form of preventive evaluation is done regularly by pharmacists and health workers at Jetis Health Center Yogyakarta.
