EKSPRESI mRNA VEGF PADA KARSINOMA OVARIUM SEROSUM DERAJAT TINGGI DAN MUSINOSUM DIHUBUNGKAN DENGAN USIA

Karsinoma ovarium terdiri atas 4 tipe utama yaitu serosum, musinosum, endometrioid, dan clear cell. Setiap tipe memiliki karakteristik klinik, molekuler, dan biologik yang berbeda. Salah satu penanda yang menjanjikan sebagai agen terapi karsinoma ovarium adalah anti VEGF. VEGF berperan dalam angiogenesis karsinoma ovarium, tetapi penelitian yang menitikberatkan ekspresi VEGF pada karsinoma ovarium tipe serosum derajat tinggi dan musinosum masih belum jelas. Tujuan penelitian ini adalah mengetahui tingkat ekspresi mRNA VEGF antara ovarium normal, karsinoma serosum derajat tinggi, dan musinosum, kemudian dihubungkan dengan usia. Metode: Penelitian ini menggunakan jaringan blok parafin pasien karsinoma ovarium serosum derajat tinggi dan musinosum serta ovarium normal sebagai kontrol, masingmasing sebanyak 25 sampel. Karsinoma ovarium dibagi berdasarkan kelompok usia (< 50 tahun dan >50 tahun). Ekspresi mRNA VEGF diperiksa dengan Quantitative polymerase chain reaction (qPCR). Analisis menggunakan uji beda Mann Whitney dan uji korelasi Spearman. Hasil: Analisis menunjukkan terdapat perbedaan ekspresi mRNA VEGF pada karsinoma serosum derajat tinggi dan musinosum dibandingkan dengan ovarium normal (p<0,001, p=0,019), sedangkan ekspresi mRNA VEGF antara karsinoma serosum derajat tinggi dan musinosum tidak berbeda bermakna (p=0,712). Tidak ada hubungan ekspresi mRNA VEGF pada karsinoma musinosum dan serosum derajat tinggi antara usia < 50 tahun dan >50 tahun (p=0,7, p=0,521). Kesimpulan: Ekspresi mRNA VEGF lebih tinggi pada karsinoma ovarium, baik serosum derajat tinggi maupun musinosum, dibandingkan ovarium normal, tetapi tidak berbeda bermakna antara karsinoma serosum derajat tinggi dengan musinosum. Ekspresi mRNA VEGF pada karsinoma ovarium tidak berhubungan bermakna dengan usia.
Background : Ovarian carcinomas account for 90% of ovarian malignancy. It consists of four major categories : serous, mucinous, endometrioid and clear cell subtype. Each category has different clinical, molecular, and biological characteristics. One of the promising molecular markers to be a therapeutic agent is anti-VEGF. VEGF play a role in angiogenesis of ovarian carcinoma, however, investigations published that emphazised VEGF mRNA expression in ovarian carcinomas have not been elucidated. The aim of this study is to determine differences in gene expression levels of VEGF between high grade serous and mucinous carcinoma of ovary and their correlation with the age. Material and Methods: This cross sectional study involve formalin fixed paraffinembedded tissues diagnosed as ovarian carcinoma which consists of high grade serous, mucinous, and normal ovaries as control, each consists of 25 samples. The carcinomas were devided by age groups (< 50 years and > 50 years). Quantitative Polymerase Chain Reaction technique was used to measure the level of VEGF in tissues, followed by Mann-Whitney test to analyse VEGF gene expression levels difference between two subtype of ovarian carcinomas and Spearman test to analyse their correlation with age. Result: There was a significant difference of VEGF mRNA expression between ovarian carcinomas (high grade serous and mucinous) and normal ovaries (p<0,001,p=0,019). There was no significant difference of VEGF mRNA expression between high-grade serous and mucinous ovarian carcinoma (p=0,712). There was no significant corellation of VEGF mRNA expression between ovarian carcinomas (mucinous and high-grade serous subtypes) and age groups (p=0,7 and p=0,521). Conclusion: Expression of VEGF mRNA is higher in ovarian carcinomas than normal ovaries, but not significantly different between high-grade serous and mucinous carcinomas. Expression of VEGF mRNA is also not correlate with age.
