The main point of the study is how the implementation of integrated library information system version 3 at Gadjah Mada University Library. The study is aimed at assessing the running of SIPUS version 3.0. in the transaction of circulation at GMU Library and evaluating the problems facing the library in relation to SIPUS version 3.0. being implemented. The theories directly applied as the basis for this study is especially related to the concepts of information system development and internal controling system. The data were gathered using methods of interviews, observations and questionnaires. The interviews were held with the up to mid levels of management; while observations were held at activities related to the library information system (SIPUS) version 3.0. and questionnaires distributed managers and some users to support the answers of interviews. The analysis method was qualitative one. The results show that the implementation of integrated library information system (SIPUS) version 3.0. has run well. The circulation service process and membership service at GMU Library have run well. However, there should be asset accountability control through stock opname done regularly to support the information validity at the library information system (SIPUS) version 3.0. the library information system (SIPUS) version 3.0. is still limited to serve the circulation and membership. The barriers existing at GMU Library are related to the policy, the less optimum of library information system (SIPUS) version 3.0. application, and less training and education for users on the library information system and human resource management as network administrator and technical support that are not sufficient yet.
