ANALISIS FAKTOR PENYEBAB MENUNGGAKNYA PEMBAYARAN PREMI PESERTA JAMINAN KESEHATAN NASIONAL MANDIRI PEKERJA BUKAN PENERIMA UPAH DI KOTA BAUBAU (SEBUAH STUDI KASUS DI KOTA BAUBAU)

Latar Belakang: Peserta jaminan kesehatan nasional mandiri pekerja bukan penerima upah merupakan peserta JKN yang rentan untuk menunggak membayar premi. Berdasarkan data BPJS sekitar 9 juta peserta JKN Mandiri PBPU atau sekitar 47,33% tidak teratur membayar premi. Permasalahan ini banyak terjadi pada kelompok sektor informal atau Pekerja Bukan Penerima Upah (PBPU ) dan bukan pekerja (BP). Aspek perluasan cakupan kepesertaan dan kesinambungan iuran khususnya peserta PBPU dan BP merupakan tantangan besar yang harus dihadapi oleh BPJS kesehatan. Data laporan BPJS Kesehatan Cabang Baubau tahun 2015 peserta JKN Mandiri mencapai 6.816 peserta dan belum melunasi iuran mencapai 1.761 peserta. September 2016 peserta JKN Mandiri Kota Baubau yang menunggak pembayaran iuran mencapai 3000 peserta. Tujuan Penelitian: Menganalisis faktor penyebab menunggaknya pembayaran premi peserta JKN Pekerja Bukan Penerima Upah di Kota Baubau. Metode: Penelitian ini menggunakan metode deskriptif, dengan rancangan penelitian mixed method : concurrent embedded (studi kasus). Teknik pengambilan sampel kualitatif menggunakan purposive sampling serta pengambilan sampel kuantitatif menggunakan cluster randome sampling. Unit analisis dalam penelitian ini adalah masyarakat peserta JKN PBPU yang menunggak membayar iuran. Pengumpulan data dilakukan dengan wawancara mendalam dan kuesioner. Hasil: Terdapat delapan faktor yang mempengaruhi penunggakan premi peserta PBPU di Kota Baubau yakni karasteristik sosio demografi (pekerjaan dan pendapatan), status kesehatan, riwayat penyakit kronis dan kelas perawatan, pengetahuan, akses ke saluran pembayaran seperti tempat, jarak, waktu dan biaya transportasi ke tempat pembayaran premi dan pengalaman pemanfaatan layanan kesehatan dan lupa Kesimpulan: Penunggakan premi oleh peserta JKN PBPU di Kota Baubau masih tinggi karenanya diperlukan terobosan dan strategi yang tepat untuk mencegah dan mengurangi peserta PBPU yang menunggak premi.

Background: National Health Insurance�s (NHI) as informal sectors member are facing difficulties in paying the contributions are vulnerable NHI participants to premiums payment of arrear. Based on data from approximately 9 million informal sector participants or approximately 47.33% do not regularly pay premiums. This problem is more common in the informal sector workers (PBPU) and non-workers (BP). The aspect of expanding the coverage of membership and contribution sustainability, especially participants PBPU and BP is a major challenge that must be faced by BPJS Kesehatan. Objective: Analyzing the contributing factors for the unpaid premiums of participants of informal workers (JKN PBPU) in Baubau City. Methods: This study used a descriptive method, design research is mixed method: concurrent embedded (case study). The sampling technique used qualitative purposive sampling and quantitative sampling using cluster randome sampling. The unit of analysis in this study is that people who are in unpaid informal sector participants pay dues of NHI. The data was collected by in-depth interviews and questionnaires. Results: There are 8 factors that cause PBPU participants in arrears in paying premiums in Baubau City are socio-demographic categorization because the types of jobs that are not earning salary with earnings below the UMR, forgot, dissatisfied with the health services provided, limited access to premium payment channels, better health status (Healthy), class restrictions, health seeking behavior and knowledge are the causes of PBPU participants delinquent paying premiums. Conclusions: Unpaid premiums by JKN PBPU participants in Baubau City are still high, therefore needed a breakthrough and the right strategy to prevent and reduce to PBPU participants who are in arrears of premiums.
