Hubungan Penggunaan Jangka Panjang Sepatu Militer Pada Prajurit Batalyon Infanteri TNI-AD Terhadap Angka Kejadian Flatfoot Di Lingkungan KODAM IV/Diponegoro

2017 | Tesis-Spesialis | SP ORTHOPAEDIK DAN TRAUMATOLOGIPendahuluan: Pes-planus (flat foot) merupakan salah satu deformitas pada kaki yang sering dijumpai. Berbagai faktor terbukti berkorelasi dengan kejadian Flatfoot, diantaranya penggunaan alas kaki, usia tua, laki-laki, ras Afrika Amerika, sosial ekonomi, dan status kesehatan. Tujuan dari penelitian ini adalah untuk mengevaluasi kejadian flatfoot pada populasi tentara infrantri yang menggunakan sepatu boot militer. Metode: Penelitian potong lintang ini dilakukan pada kelompok tentara infantri di Lingkungan KODAM IV/Diponegoro. Subjek penelitian terbagi menjadi 2 kelompok yaitu Group A (400 subjek) yang menggunakan sepatu boots militer standard tanpa insole arkus (Basic Combat Training Camp) dan Group B (400 subjek) yang memakai sepatu boots militer standard dnegan insole arkus (Battalion infantry camp). Parameter penilaian morfometri kaki dihitung menggunakan Arch Index dan dievaluasi menggunakan J-Image Software. Analisis statistik berdasarkan hasil analisis SPSS. Hasil: Terdapat 800 subjek yang diikutsertakan dengan usia, berat badan, tinggi badan, dan tahun pengabdian tidak berbeda bermakna seacra statistic. Usia rata-rata adalah 30,91 tahun. Dari 1600 kaki yang diperiksan, 39,8 % diantaranya memiliki arkus kaki rendah dan 60.3% memiliki morfologi kaki yang normal. Penggunaan sepatu booth dengan tipe berbeda mempunyai hubungan yang kuat secara statistic terhadap kejadian flatfoot (p 0.00). Penelitian ini menunjukkan perbedaan yang signifikan secara statistik pada Foot Length /FL, toe length /TL, Ball of Foot Lenght/BFL, Outside Ball of Foot/OBF, Ball of Foot Width /BFW, Heel Width/HW, Plantar Arch Width/PAW, Circumferences Ball Girth/CBG,and Ball Angle/BA, Arch Angle/AA, Midfoot Footprint Area/MFA, Whole Footprint Area/WFA, Arch Index/ AI. Penggunaan sepatu boot militer standard tanpa insole kaki meningkatkan kemungkinan terjadinya flatfoot sebanyak 6.5 (4.505-8,512). Faktor lain yang berpengaruh adalah Body Mass Index dan Ras/Etnik (p<0,05). Dari analisis korelasi pearson, PAW, AA, MFA and WFA berkorelasi kuat terhadap Flatfoot(R>0,75). Sedangkan FL, NE, BFL, BFW, HW, CBG, OBF and BA hanya memiliki korelasi lemah (R=0,1-0,25). Kesimpulan: Penelitian ini menunjukkan pemakaian sepatu boots militer tanpa insole arkus kaki secara signifikan meningkatkan risiko terjadinya deformitas flat foot. PAW, AA, MFA dan WFA memiliki korelasi kuat terhadap flatfoot.
Introduction: Pes-planus (flat foot) is one of the most common conditions observed in adult health practice. Several factors have been suggested to cause flatfoot deformity in adult population including daily footwear. Moreover, the use of daily improper footwear can increase the risk of foot deformity. Literature described that the use of military standard boots act as predictors of development flatfoot deformity. The objective of this study was to evaluate the incidence of flatfoot in infantry population wearing military boots. Methods: This was a cross sectional study in infantry army population in Kodam IV Diponegoro. Subject population was divided into 2 groups; Group A was 400 subjects using military standard boots without arch foot insole (Basic Combat Training Camp) and Group B was 400 subjects using military standard booth with arch foot insole (Battalion infantry camp). Several parameters of those eligible subjects footprint morphometric was taken using Arch Index and evaluated using J-Image Software. The statistical analysis was conducted with SPSS. Results: There were 800 subjects were reviewed and it showed that age, body weight, body height and years of service was stastically similiar between both groups (p>0,05). The mean age of the subject was 30,91 years old. From 1600 feet that were examined, it showed that 39,8 % had lower foot arch and 60,3 % had normal foot morphology. It was found that there was significant correlation (p 0.000) between the incidence of flat foot and the use of different type military boots. Further evaluation show different of foot parameter Foot Length /FL, toe length /TL, Ball of Foot Lenght/BFL, Outside Ball of Foot/OBF, Ball of Foot Width /BFW, Heel Width/HW, Plantar Arch Width/PAW, Circumferences Ball Girth/CBG,and Ball Angle/BA, Arch Angle/AA, Midfoot Footprint Area/MFA, Whole Footprint Area/WFA, Arch Index/ AI. group. The use of military standard booth without arch foot insole will increase the probability of the flatfoot deformity by 6.5 (4.505-8,512) based on statistical analysis. Conclusion: This study showed that use of military standard boots without arch foot insole was significantly increase the risk of flat foot incidence. PAW, AA, MFA and WFA were has a strong correlation with the incidence of flatfoot.
