ABSTRACT Realization of decentralized economic political system in Indonesia has encouraged local governments to distribute health assurance scheme for communities in the area, including the city of Magelang who began organizing local health insurance (jamkesda) starting in 2013. In 2014, the government of Indonesia enacted a national health security system which requires a wide range of health coverage that is distributed by various parties to be integrated into the national health security system maintained by the Agency organizer of social security (BPJS) health, including jamkesda the city of Magelang. This research aims to know the jamkesda implementation and its impact on improving access to health services for the community as well as the formulation of a policy that has been conducted by the government of the city of Magelang in the face of jamkesda program integration into the national health insurance system. This research is based on the theory of policy implementation of Edward III who explained that communication, resources, disposition and bureaucratic structure affects the success of policy implementation; Aday access theory which states that the community's satisfaction, the availability and utilization of health services is an aspect that is used in assessing the increase in access to public health services; as well as the possibility of a financial, political and administrative feasibility as a criterion alternative assessment policies according to Patton and Sawicki. This research was conducted in the city of Magelang by using descriptive qualitative approach. This research includes the informant twenty-two people from the government, one person from the BPJS health and fifteen participants jamkesda. Methods undertaken in data collection comprises pre survey, observation, in-depth interviews and documentation. Data analysis was done with the reduction of the data, the presentation of data, and the withdrawal of the conclusion. While triangulation of sources was used to test the validity of research data. The results of this research show that the implementation of jamkesda Magelang city has been running well, the problem is only on membership. Implementation of the jamkesda impact on the increased access of society towards health services related to access to information, access to health facilities, cost of access and utilization of health services. Socializing still needs to be improved in order to provide clearer information for the community. The attitude of the government towards the integration of support because it has become a liability for the government of the city of Magelang, and forms of support for the achievement of national health insurance. Currently, the formulation of policies for the integration of jamkesda until the preparation phase i.e. determination of recipient dues (PBI) and budget. Although the fiscal capacity of Magelang city being able to integrate all participants jamkesda but the government needs to pay attention to the efficiency of budget, mutual agreement between various stakeholders, right on target PBI and the acceptance of the community in integrating jamkesda Magelang city into the national health insurance system.
