ABSTRACT In the era of globalization, market mechanisms is increasingly dominated by organization / company that can provide or produce a superior product which is highly competitive, this situation also happensin medical world, including hospitals. With the rising rate of economic growth, accompanied by better educational level of the community, people started to seek and obtain credible health services and better quality treatment. One of the indicators is the emergence of many private hospitals is growing and developing rapidly in Indonesia, competing with government hospital, including in Cilacap city. Cilacap General Hospital as Government Hospital (RSUD) is required to have a competitive advantage in the health services, with this consideration, this thesis ismade to determine the strategic issues and formulate these strategies, through analysis of the external and internal environment. Afterwards, from the results of the analysis, this paper will identify variety of conditions that considered as strengths and weaknesses and the opportunities and threats with SWOT analysis method. From the results of SWOT analysis, we can see thatthere are strategic issues faced by hospitals Cilacap, namely, (1). Strength: Latitude Business Management with BLUD and availability of adequate infrastructure services, (2). Weaknesses: inadequate workforce that does not meet the standard and HR information is considered not“up to date”, (3). Opportunities: As a referral hospital for other hospitals around and its development cooperation with third parties, (4). Threats: The growth of private hospitals, physician practices, as well as specialist clinics around Cilacap General Hospital and other hospitals establishment (competitors). The next step is to establish a position based on the calculation of SWOT, the result Cilacap General Hospital is in quadrant I, meaning that there is a maximum strength and opportunities to be developed and considered to have a comparative advantage. This position indicates a strong organization and has opportunities, therefore the recommendationthat should be given is progressive strategy, that mean the organization is in good condition and steady so it is possible to continue to expand, to grow and to achieve maximum progress. Therefore, the recommended strategy formulation is to optimize business management flexibility (BLUD) for developing Cilacap General Hospital as Referral Hospital to other hospital around and optimizing infrastructure of hospital services to develop cooperation with third parties.
