KESESUAIAN PERESEPAN PASIEN SKIZOFRENIA DENGAN STANDAR PELAYANAN MEDIS DI UNIT RAWAT INAP RSJ PROF. DR. SOEROJO MAGELANG

Latar Belakang: Rumah Sakit Jiwa (RSJ) Prof.Dr.Soerojo Magelang merupakan rumah sakit tipe A untuk pelayanan kesehatan jiwa. Di RSJ Prof.Dr.Soerojo Magelang penyakit jiwa skizofrenia merupakan penyakit jiwa terbanyak dibanding penyakit jiwa lainnya. RSJ telah memiliki standar pelayanan medis (SPM)/ panduan praktik klinis (PPK) yang digunakan sebagai pedoman pengobatan di rumah sakit melalui peresepan. Penilaian peresepan sebagai salah satu cara untuk mengidentifikasi pengobatan rasional. Peresepan pasien skizofrenia di unit rawat RSJ Prof.Dr.Soerojo Magelang selama ini belum pernah dievaluasi dengan menggunakan indikator WHO, 1993 yaitu pada indikator peresepan dan evaluasi kesesuaian peresepan terhadap SPM/PPK RSJ tahun 2016. Tujuan: untuk mengetahui kerasionalan peresepan pasien skizofrenia berdasar indikator peresepan dari WHO, 1993; mengetahui kesesuaian peresepan pasien skizofrenia dengan SPM; mengetahui penerapan SPM dan mengetahui ketersediaan obat pasien skizofrenia di Instalasi Farmasi RSJ Prof.Dr.Soerojo Magelang. Metode Penelitian: penelitian ini menggunakan jenis penelitian deskriptif dengan rancangan studi kasus. Data penelitian berupa data kuantitatif yang diperoleh dengan observasi dokumen secara retrospektif yaitu 300 resep dalam rekam medis pasien skizofrenia di Instalasi Rekam Medis dan data ketersediaan obat pasien skizofrenia sesuai SPM/PPK RS di Instalasi Farmasi serta data kualitatif dengan wawancara mendalam dengan responden. Hasil Penelitian: berdasarkan indikator peresepan penggunaan obat sudah cukup rasional yaitu rata-rata item obat per resep 3,18; persentase penulisan resep dengan nama generik 91,3%; persentase resep dengan injeksi 1,3%; dan persentase peresepan obat sesuai dengan formularium 100%. Persentase kesesuaian peresepan pasien skizofrenia dengan SPM/PPK sebesar 83,6% dan masih perlu ditingkatkan. Peresepan pasien skizofrenia yang belum sepenuhnya sesuai SPM/PPK salah satu karena faktor dari penerapan SPM/PPK belum optimal dalam hal sosialisasi ke prescriber dan evaluasi SPM/PPK di RSJ Prof.Dr.Soerojo Magelang. Ketersediaan obat sudah cukup baik dengan 100% obat tersedia di Instalasi Farmasi. Kesimpulan: kesesuaian peresepan pasien skizofrenia belum sepenuhnya sesuai dengan SPM/PPK karena belum optimalnya sosialisasi SPM/PPK ke prescriber dan evaluasi terhadap SPM/PPK di RSJ Prof.Dr.Soerojo Magelang.

Background: Prof. Dr. Soerojo Psychiatric Hospital (RSJ) is type-A hospital for mental health services. In RSJ Prof. Dr. Soerojo, schizophrenia is the most common mental illness in comparison with other mental illnesses. Psychiatric Hospital has standard medical service (SPM)/ clinical practice guidelines (PPK) used as hospital treatment guidelines through prescription. The assessment of prescription is one method to identify rational treatment. The prescription of schizophrenic patients in inpatient unit of RSJ Prof. Dr. Soerojo Magelang has never been evaluated using WHO indicator, 1993, regarding the prescription and evaluation of prescription conformity to SPM/PPK RSJ of 2016. Objective: To determine the rationale of prescription for schizophrenic patients based on WHO,1993 drug use indicator, to determine the conformity of prescription for schizophrenic patients to medical service standards, to determine implementation of medical service standard and to determine drug availability for patients with schizophrenia in pharmacy installation RSJ Prof.Dr.Soerojo Magelang. Method: The present research employed descriptive type with case study design. The research data were in the form of quantitative data obtained by retrospective document observation 300 prescription documents retrospectively in the medical records of schizophrenic patients and drug availability data the Pharmaceutical Installation and qualitative data in-depth interviews with respondents. Results: The prescription usage indicator is considerably rational, i.e. drug item per prescription is 3.18. The percentage of prescription writing under generic name is 91.3%. The percentage of prescriptions with injections is 1.3%, and the percentage of drug prescription is in conformity with formulary of 100%. The percentage of conformity of schizophrenic patients prescription to SPM/PPK is 83.6%, and it still needs some improvement. The prescription for schizophrenic patients which is not fully in conformity to SPM/ PPK is due to the factor of non-optimal SPM/PPK application, in terms of the dissemination to prescribers and evaluation at RSJ Prof.Dr.Soerojo Magelang. The availability of drugs is quite good, with 100% of the drugs are available at Pharmaceutical Installations Conclusion: The prescription to schizophrenic patients does not fully conform to SPM/PPK due to non-optimal socialization of SPM/PPK to prescribers and evaluation of SPM/PPK at RSJ Prof.Dr.Soerojo Magelang.
