ASSESSMENT OF HYPOTHYROIDISM SCREENING QUESTIONNAIRE AND ITS RELATION WITH THYROID HORMONE STATUS IN ELDERLY IN BALAI PELAYANAN SOSIAL TRESNA WERDHA ABIYOSO YOGYAKARTA

2017 | Skripsi | S1 PENDIDIKAN DOKTERINTISARI Latar Belakang: Seiring dengan proses penuaan, kelenjar tiroid akan mengalami perubahan baik secara fisiologis maupun anatomis. Kelenjar tiroid akan mengalami atrofi dan fibrosis, perubahan ini akan mengakibtkan terganggunya keseimbangan hormon tiroid pada tubuh manusia. Ditambah lagi prevalensi penyakit autoimun yang menyebabkan gangguan terhadap kelenjar tiroid meningkat dengan drastis pada pasien usia lanjut. Penurunan kadar hormon tiroid atau hipotiroid akan menimbulkan banyak perubahan terhadap kinerja tubuh seorang individu. Prevalensi dari penyakit hipotiroid meningkat secara drastis pada pasien usia lanjut, namun penegakan diagnosis sering kali terlewatkan sebab gejala dari penyakit hipotiroid tidak spesifik dan sering disalahartikan sebagai proses penuaan. Tujuan: Menguji dan mengevaluasi daftar pertanyaan skrining gejala hipotiroid yang telah dikembangkan dalam bentuk kuesioner dan hubungannya terhadap level hormon tiroid pada pasien lanjut usia di Balai Pelayanan Sosial Tresna Werdha Abiyoso Yogyakarta. Metode: Jenis penelitian ini adalah deskriptif analitik observasional dengan rancangan cross-sectional. Pemilihan subjek penelitian menggunakan metode consecutive sampling pada subjek yang memenuhi kriteria inklusi dan eksklusi. Hasil: Dari 48 sampel yang memenuhi kriteria inklusi dan eksklusi, didapatkan 2 peserta hipotiroidisme sekunder (4,2%), 9 isolated hypothyroxinemia (18,7%), 30 eutiroid (62,5%) dan 7 hipertiroidisme subklinis (14,9%). Hasil uji menunjukkan bahwa status hormon tiroid tidak memiliki pengaruh yang signifikan terhadap skor skrining gejala hipotiroid pada usia lanjut (nilai p=0,701) Kesimpulan: Status hormon tiroid tidak memiliki pengaruh yang signifikan terhadap skor skrining gejala hipotiroid pada usia lanjut. ABSTRACT Background: As human ages, thyroid gland will undergo both anatomical and physiological changes. Thyroid gland will undergo atrophy and fibrosis these changes will disturb the thyroid hormone level in human body. Furthermore the prevalence of autoimmune disease that causes progressive destruction of thyroid gland is greatly increased in elderly patient. The decrease of thyroid hormone (hypothyroidism) will cause many changes in human body. The prevalence of hypothyroidism increases greatly in elderly, but the diagnosis is often missed because of the non specific sign and symptoms, and neglected because it is often misinterpreted as an aging process. Objective: To test and evaluate the developed hypothyroidism screening signs and symptoms list (in the form of a questionnaire) and its relation with thyroid hormone level at Balai Pelayanan Sosial Tresna Werdha Abiyoso Yogyakarta Method: This study is a descriptive and analytical observational with cross sectional design. Selection of research subjects using consecutive sampling method in subjects who fullfil the inclusion and exclusion criteria. Results: From 48 samples that fulfill the inclusion and exclusion criteria, there are 2 samples are classified as secondary hypothyroidism (4,2%), 9 samples are classified as isolated hypothyroxinemia (18,7%), 30 samples are classified as euthyroid (62,5%), and 7 samples are classified as subclinical hypothyroidism (14,6%). The test results indicate that thyroid hormone status doesn't have significant effect towards the hypothyroidism screening questionnaire score in elderly subjects (p value=0,701). Conclusion: There is no significant statistical finding between thyroid hormone status and hypothyroidism screening questionnaire score in elderly population.
