UPAYA KELENGKAPAN PENGISIAN LAPORAN OPERASI DAN CATATAN ANESTESI/SEDASI PASIEN SECTIO CAESAREA TERKAIT PERSIAPAN AKREDITASI KARS 2012 DI RSUD BAGAS WARAS KLATEN

Latar Belakang : RSUD Bagas Waras Klaten akan melaksanakan akreditasi KARS 2012. Pada Standar KARS 2012 terdapat kelompok Pelayanan Anestesi dan Bedah yakni Standar PAB.5.2 terkait catatan anestesi dan Standar PAB.5.2 terkait laporan operasi. Sehingga diperlukan pendokumentasian yang lengkap karena itu merupakan salah satu aspek penilaian akreditasi guna meningkatkan mutu rekam medis. Formulir laporan operasi dan catatan anestesi termasuk rekam medis pasien bedah. Sectio caesarea termasuk tindakan bedah beresiko tinggi berhubungan dengan kesehatan ibu dan anak, serta selama pembedahan dilakukan tindakan anestesi guna mempertahankan hidup penderita. Tujuan : Mendeskripsikan kelengkapan dalam pemenuhan Standar KARS 2012, dan upaya dalam memenuhi kelengkapannya. Metode : Jenis penelitian ini menggunakan penelitian deskriptif dengan pendekatan kualitatif. Teknik pengumpulan data dengan wawancara, observasi, dan studi dokumentasi. Pengambilan sampel objek menggunakan random sampling. Uji keabsahan data dengan triangulasi sumber dan teknik. Hasil : Persentase analisis Standar PAB.7.2 untuk laporan operasi 86,20% dan Standar PAB.5.2 untuk catatan anestesi pasien sectio caesarea 100% serta tiap standarnya sudah terpenuhi. Adanya penyebab ketidaklengkapan 100% pada laporan operasi. Upaya dalam memenuhi kelengkapan adalah kesadaran dokter melengkapi rekam medis, perawat mengingatkan dokter, Kepala RM berkoordinasi dengan Kepala Ruang, analisis kuantitatif pada formulir yang diabadikan saja, formulir sesuai kebutuhan dan SPO analisis kuantitatif tetapi belum disahkan direktur. Kesimpulan : Persentase analisis dan pemenuhan Standar PAB.7.2 untuk laporan operasi, Standar PAB.5.2 untuk catatan anestesi pasien sectio caesarea sudah terpenuhi dan upaya yang dilakukan dalam memenuhi kelengkapan dapat dilihat berdasarkan aspek man, materials dan method. Background: RSUD Bagas Waras Klaten will carry out KARS 2012 accreditation. In KARS Standard 2012 there are a group of Anesthesia and Surgical Services namely PAB.5.2 Standard related to anesthesia records and Standard PAB.7.2 related to operation report. So that it is required a complete documentation because it is one aspect of accreditation assessment in order to improve the quality of medical record. An operation report form and anesthesia records include medical records of surgical patients. Sectio caesarea includes high-risk surgical action relating to maternal and child health, also during surgery anesthesia is performed to sustain the patient's life. Objective: Describe the completeness in fulfillment of KARS Standard 2012, and effort in fulfilling its completeness. Method: This type of research used descriptive research with qualitative approach. Technique of collecting data by interview, observation, and study of documentation. The Sampling of objects using random sampling. The validity test by using source triangulation and technique. Result: Percentage analysis of Standard PAB.7.2 for operation report 86.20% and Standard PAB.5.2 for anesthesia records of sectio caesarea patient 100% and each standard has been met. The cause of 100% incompleteness in the operation report. Efforts in fulfilling the completeness are the awareness of the doctor completing the medical record, the nurse remind the doctor, the Head of RM coordinate with the Head of Space, quantitative analysis on the enshrined form only, the form as needed and the SPO quantitative analysis but it is not authorized by the director yet. Conclusions: Percentage of analysis and compliance with PAB.7.2 Standard for operation report, Standard PAB.5.2 for anesthesia records of sectio caesarea patient has been fulfilled and efforts made in fulfilling the completeness can be seen based on aspects of man, materials and method.
