REDESAIN FORMULIR DAN MAP REKAM MEDIS DI BALAI PENGOBATAN DAN RUMAH BERSALIN MUJAHIDAH BANTUL YOGYAKARTA

Latar Belakang: pelaksanaan rekam medis di Balai Pengobatan dan Rumah Bersalin Mujahidah sudah berjalan dengan baik. Formulir rawat jalan dan kebidanan sudah ada, akan tetapi masih ada beberapa kekurangan. Berdasarkan wawancara yanag dilakukan dengan petugas dikatakan bahwa map atau pelindung formulir yang ada masih perlu diperbaiki karena terdapat item yang salah, seperti nomor identitas harusnya diganti alamat. Formulir rawat jalan juga perlu diperbaiki karena tidak terdapat item edukasi dan alergi. Selain itu, map dan formulir masih disatukan dengan staples sehingga agak menyulitkan ketika formulirnya habis dan harus ditambahkan. Berdasarkan wawancara tersebut juga dikatakan bahwa perlunya dibuat formulir kebidanan khususnya formulir observasi untuk mendokumentasikan perkembangan pasien. Tujuan: Perancangan ini bertujuan untuk mendesain formulir rawat jalan dan formulir kebidanan khususnya formulir observasi serta mendesain map untuk melindungi formulir rekam medis. Proses Perancangan: Perancang melakukan studi dokumentasi, observasi, dan wawancara untuk mengetahui permasalahan yang ada sebagai tahap menentukan ide perancangan. Setelah itu perancang melakukan identifikasi kebutuhan map dan formulir rekam medis di Balai Pengobatan dan Rumah Bersalin Mujahidah untuk mengetahui map dan formulir yang sesuai dengan kebutuhan. Selanjutnya perancang mendesain dua alternatif map serta formulir rekam medis rawat jalan dan formulir kebidanan khususnya formulir observasi. Perancang kemudian mengajukan hasil rancangan ke Balai Pengobatan dan Rumah Bersalin Mujahidah untuk dilakukan pemilihan dan diberi masukan. Desain terpilih dilakukan revisi sesuai dengan memperhatikan masukan dari informan. Setelah revisi, perancang menunjukkan hasil yang telah direvisi kepada informan di Balai Pengobatan dan Rumah Bersalin Mujahidah untuk disetujui. Hasil: Rancangan terpilih adalah desain map rekam medis alternatif pertama dengan jenis kertas ivory 260 gram ukuran 32,5x48 cm serta formulir rekam medis rawat jalan alternatif pertama dan formulir kebidanan khususnya formulir observasi dengan kertas HVS 80 gram ukuran A4.

Design Process: Designer conductted study documentation, observation,and interview to know the problem is as stage determine the idea of. After that Designer do identification the needs of the folder and recorded in the form of medical treatment and the delivery room Mujahidah to know the folder and form in accordance with the needs of. Next designer designing two alternative folder as well as record form of medical outpatient and form obstetrics in particular form observation. Designer then ask designed to the treatment and the delivery room Mujahidah to do the selection and given feedback. The design elected done revision in accordance with the pay attention to the input from the informant. After revision, Designer show result that has been revised to the informant in the treatment and the delivery room Mujahidah approved. Results: the draft elected is the design of the folder record medical alternative first with the kind of paper ivory 260 gram size of 32,5x48 cm as well as record form of medical outpatient alternative first and form obstetrics in particular form observation with the paper HVS 80 gram of size A4.
