FAKTOR PENYEBAB BELUM TERLAKSANANYA SISTEM PEMUSNAHAN REKAM MEDIS DI PUSKESMAS WONOSARI 2

Latar belakang : Penyusutan merupakan salah satu sarana penting untuk mengatasi masalah bertumpuknya arsip-arsip yang tidak mempunyai nilai guna lagi. Arsip-arsip yang tidak mempunyai nilai guna lagi sebaiknya dimusnahkan agar tersedia tempat penyimpanan dan fasilitas pemeliharaan yang lebih baik terhadap arsip-arsip yang masih mempunyai nilai guna. Arsip yang masih digunakan terdapat di rak rekam medis yang diletakkan di samping dengan tempat duduk tunggu pasien. Letak yang kurang tepat ini bisa menyebabkan kebocoran informasi pasien. Kondisi ini yang melatarbelakangi peneliti ingin mengetahui lebih jauh apa saja yang menjadi faktor belum terlaksananya sistem pemusnahan rekam medis di Puskesmas Wonosari 2. Tujuan : Mengetahui faktor penyebab belum terlaksananya sistem pemusnahan rekam medis di Puskesmas Wonosari 2. Metode penelitian : Penelitian ini menggunakan metode deskriptif dengan pendekatan kualitatif dan rancangan penelitian studi kasus (case study). Teknik pengumpulan data melalui observasi, wawancara, dan studi dokumentasi. Hasil : Di Puskesmas Wonosari 2 belum pernah melakukan pemusnahan, namun pernah melakukan pemilahan berkas rekam medis aktif dan inaktif. Berkas rekam medis inaktif diletakkan di depan pintu masuk Puskesmas Wonosari 2. Faktor yang menyebabkan belum terlaksananya pemusnahan ditinjau dari 5 M : man, money, method, material, machine. Dari segi man, terdapat kurangnya petugas rekam medis dan petugas rekam medis tidak sesuai dengan kompetensinya, method : belum terdapat standar operasional dan kebijakan yang mengatur mengenai pemusnahan rekam medis, machine : belum adanya alat atau incenator untuk memusnahkan berkas rekam medis, material : belum adanya berita acara, daftar pertelaan untuk pemusnahan di Puskesmas Wonosari 2. Kesimpulan : Kegiatan pemusnahan belum dilakukan dikarenakan petugas belum mengetahui mengenai pemusnahan dan belum ada standar operasional. Solusi yang dirokemdasikan adalah pembuatan standar operasional dan sosialisasi mengenai pemusnahan.

Background: Depreciation is one of the important means to overcome the problem of unused archives accumulation. The unused archives that do not have any value should be destroyed in order to give more available storage and maintenance facilities for the useful archives. The useful archives are placed in a medical records shelf in front of the door beside the seat of waiting patients. This improper location of the a medical records archives can lead the leakage of patient information. The background of this research was to determine the factors caused non-implementation of medical record destruction system in Community Health Centre Wonosari 2. Objective: to determine the factors caused non-implementation of medical record destruction system in Community Health Centre Wonosari 2. Methods: This study used descriptive method with qualitative approach and case study design. The data collecting technique used were observation, interviews, and documentation. Results: A destruction system of the unused archives had not been done at Community Health Centre Wonosari 2, they only separating the active and inactive medical record file archives. The inactivated medical record file was placed in front the entrance door at Community Health Centre Wonosari 2. The factors contributing to the implementation of archive destruction were evaluated according to 5 M: man, money, methods, materials, machine. In terms of man, there was a lack and incompetence of medical records staff, method: there had been no operational standards and policies of the medical records destruction, machine: the lack of tools or incenator to destroy the medical record file, material: the lack of news events, a list of descriptions for extermination at Community Health Centre Wonosari 2. Conclusion: A destruction system of the unused archives has not been implemented because a limited knowledge of the staff and there was not an operational standards for it. The recommended solutions are the arrangement of an operational standards and socialization of A destruction system of the unused archives.
