FAKTOR PENYEBAB KETIDAKLENGKAPAN PENGISIAN LEMBAR RESUME DALAM 1x24 JAM SETELAH PELAYANAN DI RSI KLATEN

Latar belakang: Sebagai fasilitas pelayanan kesehatan, rumah sakit berkewajiban dalam penyediaan berkas rekam medis untuk pelayanan pasien. Pada berkas rekam medis harus mengandung informasi yang lengkap dan akurat. Standar kelengkapan pengisian rekam medis dalam 1x24 jam setelah selesai pelayanan adalah 100%. Peneliti menemukan ketidaklengkapan pengisian lembar resume medis di RS Islam Klaten. Hal tersebut diperkuat dengan adanya laporan indikator unit rekam medis RS Islam Klaten yang menunjukkan angka 18% berkas tidak lengkap pengisiannya dalam waktu 1x24 jam setelah pelayanan selesai. Di dalam ketidaklengkapan berkas rekam medis tersebut termasuk diantaranya ketidaklengkapan pada lembar resume. Lembar resume sendiri berisikan ringkasan seluruh pelayanan yang diberikan oleh tenaga medis kepada pasien. Tujuan: Penelitian ini bertujuan untuk mengetahui proses pengisian lembar resume medis di RS Islam Klaten, untuk mengetahui tingkat ketidaklengkapan lembar resume di RS Islam Klaten, dan faktor penyebab ketidaklengkapan pengisian lembar resume dalam 1x24 jam setelah pelayanan di RS Islam Klaten. Metode: jenis penelitian adalah deskriptif kualitatif. Rancangan penelitian ini menggunakan rancangan penelitian fenomenologi. Untuk keabsahan data, peneliti menggunakan triangulasi teknik dan sumber. Subjek dalam penelitian ini adalah lima dokter dan empat sumber untuk triangulasi. Objek penelitian ini adalah seluruh lembar resume kembali dari bangsal pada tanggal 07 Maret 2017 sebanyak 63 lembar. Hasil : Berdasarkan hasil observasi proses pengisian lembar resume didapatkan hasil bahwa lembar resume kosong berada di bangsal, dokter akan mengisinya saat dokter visit ke bangsal. Berdasarkan studi dokumentasi didapatkan hasil ketidaklengkapan lembar resume medis mencapai 93,7% item tidak terisi tertinggi 57% pada item anjuran. Berdasarkan hasil wawancara penyebab ketidaklengkapan lembar resume dari faktor SDM adalah karena kesibukan dokter dan dokter yang tidak selalu berada di bangsal. Tidak adanya evaluasi terhadap ketidaklengkapan lembar resume yang menghadirkan dokter untuk memberikan masukan atau usulan kepada pihak manajemen. Tidak adanya resosialisasi terhadap prosedur pengisian berkas rekam medis dan tidak adanya rincian untuk prosedur pengisian lembar resume. Terdapat ketidakefisiensian susunan formulir resume medis yang membuat dokter mengutip kembali jawaban pada item sebelumnya. Tidak adanya reward atau apresiasi kepada dokter yang rajin mengisi lengkap lembar resume dalam 1x24 jam setelah pelayanan pasien. ABSTRACT Background: as a health care facility, the hospital has been obligated in the provision of medical record file for patient service. In the medical record file, the information that is contained must be complete and accurate. The standard of completeness of medical record fulfilment within 1x24 hours after the service is 100%. The researcher finds incompleteness of medical resume sheet fulfilment in RSIKlaten. This finding is reinforced by an indicator report of medical record unit of RSIKlaten which shows 18% of file are incomplete in the fulfilment within 1x24 hours after the service is done. In this incompleteness of medical record file, it includes incompleteness on resume sheet. The resume sheet itself contains a summary of all services provided by medical personnel to the patients. Objectives: this research aims to find out the process of medical resume sheet fulfilment in RSIKlaten, to find out the degree of resume sheet incompleteness in RSIKlaten, and the causative factor of incompleteness of resume sheet fulfilment within 1x24 hours after the service in RSIKlaten. Method:this research employed a descriptive method with qualitative approach. The design of this research used research design in phenomenology. To validate the data, the researcher employed triangulation of technique and source. Subjects in this research were five doctors and four sources to do triangulation. The objects in this research were all resume sheets restored from the ward on 07 March 2017 as much as 63 sheets. Findings:based on the observation result of the process of resume sheet fulfilment, the blank resume which were in the ward would be filled by the doctor while doing a visit to the ward. Based on study of documentation result, the incompleteness of medical resume sheet reached 93,7% highest unallocated items 57% on the recommendation. Based on the interview result, the causative factors of incompleteness of resume sheets which were from Human Resources were because of doctors�¢ï¿½ï¿½ activities and doctors who were not always in the ward. There was a lack of evaluation towards incompleteness of resume sheet that presented the doctors to give input or proposal to the management. There were no resocialization towards the procedure of medical record file fulfilment danno the detailed procedure of resume sheet fulfilment. There was inefficiency of medical resume form order that made doctors quote again the answer of the previous item. There was no reward or appreciation to doctors who diligently filled the complete resume sheets within 1x24 hours after patient service.
