PERSIAPAN AKREDITASI PUSKESMAS TERKAIT KERAHASIAAN DAN KEAMANAN REKAM MEDIS DI PUSKESMAS PAKUALAMAN YOGYAKARTA

Latar Belakang : Pelaksanaan kerahasiaan dan keamanan rekam medis merupakan salah satu standar akreditasi yang dinilai dalam Standar Akreditasi Puskesmas yang terlampir dalam Permenkes Nomor 46 Tahun 2015. Puskesmas Pakualaman Yogyakarta merupakan puskesmas yang sedang mempersiapkan akreditasi dan belum pernah dilakukan akreditasi.Oleh karena itu, persiapan akreditasi sangat diperlukan. Tujuan : Tujuan dari penelitian ini untuk mengetahui persiapan akreditasi pada kriteria 8.4.2 petugas memiliki akses infomasi sesuai dengan kebutuhan dan tanggungjawab pekerjaan dan kriteria 8.4.4 rekam berisi informasi yang memadai dan dijaga kerahasiaanya tentang identifikasi pasien, dokumentasi prosedur kajian, masalah, kemajuan pasien dan hasil asuhan. Metode :Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan rancangan penelitian fenomenologi. Subjek dalam penelitian ini adalah enam petugas rekam medis, penanggung jawab dan pimpinan puskesmas.Objek dalam penelitian ini yaitu pelaksanaan, standar operasional prosedur (SOP), dan dokumen terkait kerahasiaan dan keamanan berkas rekam medis.Teknik pengumpulan data menggunakan teknik studi dokumentasi, wawancara dan observasi.Keabsahan data menggunakan triangulasi sumber. Hasil :Persiapan Akreditasi Puskesmas Kriteria 8.4.2 yang telah dilakukan yaitu penyediaan Surat Keputusan Hak Akses Terhadap Rekam Medis dan Standar Operasional Prosedur Akses Terhadap Rekam Medis. Persiapan yang belum dilakukan yaitu format SOP yang ada belum menyesuaikan format akreditasi, isi dari SOP kurang merincikan kegiatan yang dilakukan dan pelaksanaan hak akses yang kurang optimal karena terdapat petugas yang memakai bukan username dan password petugas lain. Persiapan Akreditasi Puskesmas Kriteria 8.4.4 yang telah dilakukan yaitu penyediaan Surat Keputusan tentang Isi Rekam Medis dan Standar Operasional Prosedur Kerahasiaan Rekam Medis.Persiapan yang belum dilakukan yaitu format SOP yang ada belum menyesuaikan format akreditasi dan isi dari SOP kurang mencakup lingkup kerahasiaan dan keamanan rekam medis. Kesimpulan : Dokumen akreditasi puskesmas belum sesuai dan terdapat kegiatan yang belum dipersiapkan di Puskesmas Pakualaman Yogyakarta.

Background: Confidentiality and security of medical records are a few of accreditation standards assessed in Accreditation Standard for Puskesmasdocumented on the Regulations of Minister of Health of the Republic of Indonesia No. 46 Year 2015. Puskesmas Pakualaman Yogyakarta is a health facility prepared for its first accreditation. Thus, preparationsfor the accreditation isa crucial matter. Objective: This research aims to observe preparations of accreditation. On criteria 8.4.2, staff personnels own information access specific to requirement and job description. Criteria 8.4.4 of the record is filled with confidential information about patient identification, documentation of procedure, problems, development of patient's medical condition and result of the treatments. Methode:This research used descriptive with qualitative approach and design of phenomenology. The subject of this research is six medical record personnel and authorities of Puskesmas Pakualaman Yogyakarta. The objects in this research are, the implementation, Standard Operational Procedure (SOP), and documentation related to confidentiality and security of medical records. Data collection is commenced through documentation study technique, interview and observation. Data obtained is validated through source triangulation. Results: The preparation for accreditation of Puskesmas Pakualaman Yogyakarta which have been done on criteria 8.4.2 are the Decree of the Access Rights to the Medical Records and Standard Operating Procedures to Access Medical Records. The Accreditation Preparations that have not been done are the available format of SOP is not in accordance with the format of the accreditation, the content of SOP is less detailed to explain the carried out activities and the implementation of the access rights is less optimal because there are officers using the username and password instead of the other officers. The Accreditation Preparations of Puskesmas that have been done on criteria 8.4.4 are the provision of the Decree on Medical Record Content Standard Operating Procedures and Confidentiality of Medical Records. The Accreditation Preparations that have not been done are the available format of SOP is not in accordance with the format of the accreditation and the content of the SOP covers less of the scope of the confidentiality and the security of medical records. Conclusion: The document of Puskesmas accreditation does not meet the standard and there are some activities which are not prepared in Puskesmas Pakualaman Yogyakarta.
