PERBANDINGAN KELENGKAPAN FORMULIR RAWAT JALAN SEBELUM DAN SESUDAH IMPLEMENTASI FORMULIR BARU DI PUSKESMAS KEDUNGBANTENG

Latar Belakang :Kelengkapan berkas rekam medis merupakan hal yang penting bagi keselamatan pasien, medis, proses hukum dan asuransi kesehatan. Berkas rekam medis harus lengkap agar dapat digunakan dengan baik. Implementasi formulir merupakan salah satu cara untuk meningkatkan kelengkapan formulir. Puskesmas Kedungbanteng melakukan implementasi formulir baru yang awalnya hanya memiliki formulir rawat jalan. Formulir baru yang dibuat ada beberapa jenis formulir, sehingga peneliti ingin melihat perbandingan kelengkapan pada formulir rawat jalan sebelum dan sesudah implementasi formulir baru. Tujuan : Mengetahui perbandingan kelengkapan formulir rawat jalan sebelum dan sesudah implementasi formulir baru. Metode Penelitian :Jenis penelitian yang digunakan pada penelitian ini adalah observasional analitik dengan rancangan penelitian cross sectional. Populasi penelitian yaitu formulir rawat jalan sebelum dan sesudah implementasi, dengan sampel 50 formulir sebelum dan 50 formulir sesudah implementasi. Teknik pengumpulan data menggunakan teknik studi dokumentasi dan observasi. Hasil :Kelengkapan formulir rawat jalan sebelum implementasi formulir baru hanya mencapai 6% atau 3 formulir. Kelengkapan formulir rawat jalan sesudah implementasi formulir baru mencapai 12 % atau 6 fomulir. Hasil uji statistik menunjukan bahwa ada peningkatan pada komponen identifikasi (p_value 0,01295), ada peningkatan pada komponen laporan penting (p_value 0,001629), ada peningkatan pada komponen autentikasi (p_value 0,02293), tidak ada peningkatan pada komponen pendokumentasian yang benar (p_value 0,5502), ada peningkatan pada kelengkapan formulir sesudah implementasi dengan p_value 0,00176, dan nilai PR = 2 yang berarti kenaikan kelengkapan sebanyak 2 kali lipat. Kesimpulan :Implementasi formulir baru meningkatkan kelengkapan formulir rawat jalan di Puskesmas Kedungbanteng dengan peningkatan 2 kali lipat.

Background: The completeness of medical record file is important for patient safety, medical, health insurance and legal process. Medical record file must be complete in order to be used properly. Implementation of form is one way to improve the completeness of the form.Puskesmas Kedungbanteng do the implementation of a new form initially only form outpatient. A new form is created there are several types of forms, so that researchers would like to see a comparison of completeness on the outpatient forms beforeandafter the implementation of the new form. Objective: To know the comparison of the completeness of the outpatient forms before and after the implementation of the new form. Research method: The type of research used in this study was observational analytic study with cross sectional. Population research outpatient form before and after implementation, with a sample of 50 forms before and 50 forms after implementation. Data collection technique used documentation study and observation techniques. Results: Completeness of the outpatient forms before implementation of the new form is reaching only 6% or 3 form. Completeness of the outpatient forms after the implementation of the new form reaching the 12% or 6 fomulir. The results of statistical tests indicate that there is an increase on the component identification (p_value 0,01295), there is an increasing important reports on components (p_value 0,001629), there is an increase in the authentication components (p_value 0,02293), there is no improvement on the correct documentation of components (p_value 0,5502), there is an increase on the completeness of the form after implementation with 0.00176, p_value and PR value of 2, which means the increase completeness by as muchas2-fold. Conclusion: The implementation of the new form is improving the completeness of outpatient puskesmas in form Kedungbanteng with increased 2-fold.
