HUBUNGAN LINGKAR KEPALA DENGAN DERAJAT RUPTUR PERINEUM DI RUMAH SAKIT UMUM DAERAH WONOSARI

Ruptur Perineum adalah robeknya perineum pada saat janin lahir. Ruptur perineum merupakan penyebab terjadinya perdarahan setelah persalinan. Lingkar kepala yang besar diduga merupakan salah satu penyebab terjadinya ruptur perineum pada persalinan normal. Penelitian ini bertujuan untuk mengetahui hubungan lingkar kepala dengan derajat ruptur perineum. Jenis penelitian analitik observasional dengan rancangan cross sectional. Sampel penelitian diambil dari persalinan periode 1 Oktober 2015 sampai 30 September 2016, yang memenuhi kriteria sebanyak 181 orang Variabel bebas adalah lingkar kepala, variabel terikatnya adalah derajat ruptur perineum, dan variabel luar adalah berat badan lahir dan paritas. Pengumpulan data menggunakan lembar observasi. Analisis data menggunakan uji Spearman rank. Hasil penelitian menunjukkan bahwa ibu bersalin sebagian besar (45,3%) melahirkan bayi dengan lingkar kepala normal, sebagian besar ( 74,0%) melahirkan bayi dengan berat badan lahir normal gram, sebagian besar (55,8%) berparitas multipara, dan sebagian besar (38,1%) mengalami ruptur perineum derajat III. Lingkar kepala normal berisiko 3 kali mengalami ruptur perineum dibandingkan lingkar kepala kecil (CI 95% 1.421-9.350) dan lingkar kepala besar berisiko 33 kali mengalami ruptur perineum dibandingkan lingkar kepala kecil (CI 95% 4.036 -271.402).

Perineal rupture is the laceration of perineum when a fetus is being born. Perineal rupture is one of those causing postpartum hemorrhaging. The child’s large head circumference is assumed to be one of the causes of perineal rupture in natural childbirth. This research was aimed at identifying the correlation between head circumference and the level of perineal rupture. The type of this research was analytical observation by having a cross sectional design. Samples were taken from the delivery period October 1st, 2015 – September 30th, 2016 who meet the criteria as much as 181 respondents. The independent variable was the head circumference, the dependent variable was the level of perineal rupture, while the external variables being examined were birth weight and parity. The instruments used to collect the data were observation sheets. The data collected were then analyzed using Spearman rank test. The result of this research showed that were 45.3% has a birth to babies with normal head circumference, that were 74.0% have a baby with normal birth weight . That were 55.8% parity multiparas, and that were 38,1% experienced perineal rupture level III. The spearman rank test showed p=0,001 for the head circumference, p=0,000 for the birth weight, and p=0,022 for the parity. Normal head circumference 3 times the risk of perineal rupture than smaller head circumference (CI 95% 1.421-9.350) and a large head circumference 33 times the risk of perineal rupture than smaller head circumference (CI 95% 4.036 -271.402).
