KINERJA PETUGAS TB DALAM PENEMUAN KASUS TUBERKULOSIS DI PUSKESMAS KABUPATEN BENGKULU UTARA

Latar Belakang: Penyakit Tuberkulosis masih menjadi masalah kesehatan yang serius bagi dunia. Penyakit Tuberkulosis menyebabkan kesakitan, kematian,dan menurunnya produktifitas penderita. Meskipun berbagai upaya dilakukan untuk pengendalian Tuberkulosis, namun masih banyak ditemukan hambatan dan masalah dalam penanggulangannya. Kinerja petugas TB puskesmas merupakan salah satu yang menentukan penemuan kasus tuberkulosis, karena itu penting kiranya dilakukan penelitian tentang faktor-faktor yang berhubungan dengan kinerja petugas TB dalam penemuan kasus tuberkulosis di Puskesmas. Tujuan: Mengetahui faktor-faktor yang mempengaruhi kinerja petugas TB Puskesmas dalam penemuan kasus TB di Puskesmas Kabupaten Bengkulu Utara. Metode: Penelitian deskriptif kualitatif bertujuan untuk menggambarkan secara jelas sesuai kenyataan empiris yang terjadi di lapangan. Desain penelitian menggunakan pendekatan studi kasus. Subyek penelitian adalah petugas TB Puskesmas, Kepala Puskesmas, Pasien TB di Puskesmas, kepala desa serta Supervisor TB Kabupaten Bengkulu Utara, teknik pengambilan data melalui wawancara mendalam, observasi dan telaah dokumen. Hasil Penelitian: Kinerja petugas TB yang masih rendah dalam melaksanakan tugas pokok dan fungsinya, seperti dalam melakukan penyuluhan dan penjaringan suspek yang tidak dilakukan pada kelompok sasaran dengan resiko, kunjungan rumah dan pemeriksaan kontak penderita yang tidak dilakukan secara intensif, serta dukungan pimpinan puskesmas yang belum maksimal, masing-masing mempunyai andil terhadap kinerja petugas dalam penemuan kasus tuberkulosis. Kemampuan dan keterampilan petugas TB yang belum baik masih perlu dilakukan pelatihan dan supervisi on the job training yang lebih intensif. Pemberian imbalan dan rewards kepada petugas TB akan memberikan motivasi yang baik kepada petugas dalam penemuan kasus Tuberkulosis. Kesimpulan: Belum optimalnya kinerja petugas TB di puskesmas dipengaruhi oleh banyak faktor. Kepemimpinan mempunyai pengaruh terhadap kinerja petugas TB dalam penemuan kasus, kepemimpinan yang tegas dan pertisipasif bisa meningkatkan penemuan kasus tuberkulosis. Supervisi berperan dalam meningkatkan kemampuan dan keterampilan petugas TB dalam menemukan kasus. Imbalan yang diterima petugas TB memberikan motivasi kinerja penemuan kasus tuberkulosis di puskesmas.

Introduction: Tuberculosis (TB) continue to present as a serious health problem in the world. Tuberculosis can lead to pain, death, and productivity decline in those who suffer from it. Though the various effort has been conducted to control TB, there are still many obstacles and difficulties in its management. The performance of TB officers is a determining factor in detecting tuberculosis cases. Hence it is important to do research on factors correlating with the performance of TB officers in finding TB cases within the primary care setting (Public Health Center). Objective: To recognize the factors affecting the performance of TB officers in Public Health Center in detecting the cases within the primary health center setting in North Bengkulu District. Method: The research is a qualitative descriptive study aimed to describe the empirical facts in the field clearly. The research is designed using case study approach. The research subjects are Public Health Center TB Officer, Head of Public Health Center, TB patients in Public Health Center, head of the village and the TB supervisor of North Bengkulu District. Data was obtained through thorough interview, observation, and document analysis. Results: Performance of TB officers in carrying out their duties and functions to detect TB cases are less than optimal due to the facts that the counseling and outreaching of TB suspects were not arranged in the target groups at risk, home visits and inspection of patients contacts were not performed intensively, as well as the support of the head of Public Health Center are not maximized. The abilities and skills of the TB officers are still under performance and in need of training and supervision on the job training more intensively. Remuneration and rewards to TB officers will provide good motivation to the officer in TB case finding. Conclusion: The under-optimal performance of TB officers in Public Health Center is influenced by many factors. Leadership has an effect on officers performance in the TB case finding, as well as decisive and participative leadership could increase TB case detection. Supervision has a role in enhancing the capabilities and skills of the personnel in the TB case finding. TB officer remuneration provides performance motivation in tuberculosis case detection in health centers.
