ANALISIS PENYEBAB KETIDAKLENGKAPAN RESUME MEDIS PASIEN RAWAT INAP DI RSU AT-TUROTS AL-ISLAMY YOGYAKARTA

Latar Belakang : Resume medis merupakan ringkasan dari seluruh masa perawatan dan pengobatan selama pasien mendapatkan pelayanan di rumah sakit. Pengisian resume medis yang tidak lengkap akan menghambat penegakan diagnosa sehingga proses coding menjadi tertunda. Hal tersebut akan berakibat pada proses tagihan klaim yang menjadi terhambat karena informasi yang dibutuhkan belum tersedia. Tujuan : Mengetahui penyebab ketidaklengkapan resume medis pasien rawat inap di RSU At-Turots Al-Islamy Yogyakarta. Metode : Jenis penelitian yang dilakukan adalah penelitian deskriptif dengan pendekatan kualitatif menggunakan rancangan fenomenologi. Penelitian dilakukan di Instalasi Rekam Medis RSU At-Turots Al-Islamy Yogyakarta. Subjek penelitian, yaitu petugas rekam medis, dokter spesialis/DPJP, dan perawat bangsal. Objek penelitian, yaitu lembar resume medis pasien rawat inap. Hasil : Alur pengisian resume medis dilakukan setelah DPJP visit dan pasien dinyatakan boleh pulang. Resume medis yang paling banyak tidak diisi pada komponen laporan penting dan autentikasi. Penyebab yang mengakibatkan data identitas tidak terisi lengkap, yaitu proses pengisian oleh keluarga pasien, waktu pengisian oleh petugas, dan prioritas pendaftaran terhadap pasien. Sedangkan penyebab yang mengakibatkan data klinis dan autentikasi tidak terisi lengkap, yaitu kepadatan aktivitas DPJP, pengisian item bukan tanggungjawab DPJP, DPJP tidak melakukan visit, dan kurangnya peran perawat bangsal. Upaya untuk melengkapi resume medis, yaitu mengembalikan kepada DPJP saat jadwal poli selesai, diberikan surat teguran, sosialisasi, dan memberi kertas peringatan ketidaklengkapan. Kesimpulan : Ketidaklengkapan resume medis pada data klinis dan autentikasi disebabkan oleh DPJP sedangkan pada data identitas pasien oleh petugas rekam medis. Background : The medical resume is a concise all of period treatment and therapy during the patient get course in the hospital. Filling the medical resume incomplete will be hamper diagnosis enforcement so that the coding process to be delayed. This will result in the bill claims to be blocked because the required information is not available. Objective : Find out the cause of the incompleteness of medical resume inpatients in RSU At-Turots Al-Islamy Yogyakarta. Methods : This type of research is descriptive research with a qualitative approach using the phenomenological design. The study was conducted in the Installation Medical Record RSU At-Turots Al-Islamy Yogyakarta. The subject of research is the medical records officers, medical specialists, and ward nurses. The object of research is the sheet medical resume inpatients. Results : The medical resume is filling after the medical specialists visit is done and the patient allowed to go home. The most component that not fill in the medical resume is necessary reports and authentication. The causes of identity data uncomplete is the process of filling by the patient's family, the filling time by officers, and priority registration to patients. While the causes of clinical data and authentication uncomplete is the activity of the medical specialists, filling is not the medical specialists responsibility, the medical specialists do not visit, and less of role the ward nurses. Efforts to complete the medical resume is returned to the medical specialists when poly schedule is finish, given a letter of reprimand, socializing, and give a admonition papers. Conclusion : Incomplete medical resume on clinical data and authentication caused by the medical specialists while the identity data of patients by medical records officer.
