EFIKASI TRANSFUSI ERITROSIT PADA PASIEN ANEMIA ASIMPTOMATIK DENGAN KANKER SOLID STADIUM TERMINAL TERHADAP KESINTASAN 30 HARI di RUMAH SAKIT DR.SARDJITO YOGYAKARTA

Latar belakang Anemia karena penyakit kronik merupakan suatu kondisi yang banyak dialami pasien dengan kanker. Saat ini belum ada acuan baku yang dapat dipakai untuk praktek pemberian transfusi untuk kondisi anemia asimptomatik pada kanker stadium terminal. Pemberian transfusi darah sebagai terapi pada pasien anemia dengan kanker stadium akhir sampai saat ini masih dalam perdebatan, belum ada kesepakatan yang telah dicapai Tujuan Penelitian: untuk mengetahui adakah manfaat pemberian transfusi eritrosit pada pasien anemia asimptomatik dengan kanker stadium terminal pada pasien dengan solid tumor terhadap kesintasan 30 hari di Rumah Sakit dr.Sardjito Yogyakarta. Metode: Studi khusus berupa studi analisis kesintasan (survival analysis )pada pasien kanker stadium terminal dengan anemia asimptomatik yang memenuhi kriteria inklusi dan ekslusi yang akan dibagi kedalam dua kelompok : Kelompok yang dilakukan transfusi dan yang tidak dilakukan transfusi untuk melihat efikasi transfusi terhadap kesintasan 30 hari. Analisis statistik menggunakan Statistical Product and Service Solutions (SPSS)20.0 for Windows. Hasil Penelitian Bila berdasarkan jumlah sampel penelitian ini yang melibatkan 99 subyek, didapatkan rerata usia pasien 51,97+- 12,3 tahun, terdiri dari subyek laki-laki 45 (45%)dan perempuan 54 (54%)orang. kelompok yang mendapatkan transfusi eritrosit (50 orang) dan kelompok yang tidak mendapatkan transfusi eritrosit (49 orang). Tidak terdapat perbedaan kesintasan antara kelompok subyek yang mendapatkan transfusi (40 orang ) dibandingkan dengan kelompok non transfusi (39 orang) dengan P=0,96. Kesimpulan : Tidak terdapat perbedaan kesintasan antara kelompok subyek yang mendapatkan transfusi eritrosit dengan yang tidak mendapatkan transfusi pada pasien kanker stadium terminal dengan metastasis dengan kategori skor PaP C

Background. Anemia due to chronic disease is a condition experienced by many patients with cancer . Currently there is no standard reference that can be used for the practice of giving transfusions for asymptomatic anemic condition in the terminal stages of cancer. Giving blood transfusion in the treatment of anemic patients with end-stage cancer is still under debate , no agreement has been reached. Objective. To know wether there is any benefit of erythrocyte transfusions in aymptomatic anemia patients with terminal solid cancer for the survival in 30 days in DR.Sardjito hospital Yogyakarta. Method. Survival analysis study in the terminal stage of solid cancer patients with asymptomatic anemia with the inclusion and exclusion criteria will be divided into two groups : The group that carried out the transfusion and transfusion that was not done, to look at the efficacy of transfusions for survival in 30 days . Statistical analysis using the Statistical Product and Service Solutions ( SPSS ) 20.0 for Windows . Results. From the ninety nine samples of the study that were involved, with the mean age of patients 51.97 +- 12.3 years , comprising 45 male subjects ( 45 % ) and 54 women ( 54 % ) . Erythrocyte transfusion group ( 50 people ) and a group who did not receive transfusions of erythrocytes ( 49 ) . No difference in survival between the group of subjects who received a transfusion ( 40 ) compared with non- transfusion group ( 39 people ) at p = 0.96 . Conclusion. Based on the samples that involved in this study, it is conclude that no difference in survival between the group of subjects who received a transfusion of erythrocytes with not getting a transfusion in patients with terminal solid cancer with a note that there is still false negativity in this study due to the condition that the samples was not fully represent all the population research target.
