ANALISIS BIAYA PENGOBATAN PENYAKIT GINJAL KRONIS RAWAT INAP DENGAN HEMODIALISIS DI RSUP Dr. SARDJITO

Gangguan fungsi ginjal merupakan salah satu permasalahan utama kesehatan masyarakat Indonesia. Pada era Jaminan Kesehatan Nasional (JKN), untuk pembiayaan kesehatan pada Fasilitas Kesehatan Rujukan Tingkat Lanjutan (FKRTL) diberlakukan standar tarif Indonesian Case Based Group (INA-CBGs), tetapi seringkali biaya riil lebih besar dari tarif INA-CBGs. Penelitian ini bertujuan untuk mengetahui rata-rata biaya pengobatan pasien penyakit ginjal kronis (PGK) rawat inap yang menjalani perawatan hemodialisis serta untuk mengetahui komponen biaya yang paling berpengaruh terhadap besarnya tarif rumah sakit, faktor-faktor yang mempengaruhi besarnya biaya pengobatan dan perbedaan antara biaya riil dengan tarif INA-CBGs. Penelitian ini merupakan penelitian analitik cross-sectional dengan perspektif rumah sakit dan metode pengambilan data secara retrospektif. Subjek penelitian adalah pasien PGK rawat inap dengan hemodialisis yang memenuhi kriteria inklusi. Pada penelitian ini terdapat subjek sebanyak 47 episode rawat inap. Analisis karakteristik pasien menggunakan statistik deskriptif, faktor yang mempengaruhi biaya rumah sakit menggunakan korelasi Spearman serta perbedaan tarif rumah sakit dan INA-CBGs menggunakan one sample t test. Hasil penelitian menunjukkan rata-rata biaya riil pengobatan pasien PGK rawa inap dengan hemodialisis dengan tindakan operatif per pasien per episode rawat inap sebesar Rp. 23.732.520,02 (SD Rp. 19.142.379,09) dan non operatif sebesar Rp. 12.800.910,61 (SD Rp. 6.409.290,00). Pada kelompok operatif komponen biaya terbesar adalah biaya tindakan medis operatif sebesar 29,39% dan pada kelompok non operatif biaya yang terbesar pada biaya pelayanan penunjang medis sebesar 27,12% . Faktor yang mempengaruhi biaya pengobatan pasien ginjal kronik rawat inap dengan hemodialisis adalah komorbid, frekuensi HD dan LOS. Perbedaan antara biaya rill dan tarif INA-CBGs terdapat pada kelompok N-4-10-II Kelas I; N-4-10-II Kelas II; N-4-10-III; N-4-10-I Kelas I Selisih tarif rumah sakit dengan tarif INA-CBGs bulan September 2014-Agustus 2015 sebesar Rp. 225.632.939,96 (29,47% dari total biaya).

Impaired renal function is one of the main health problems which happens in Indonesia. When National Health Insurance (NHI) was implemented, the health financing given at the Referral Health Facility was based on the tariff of Indonesian Case Based Group (INA-CBGs). However, most often the real cost is much bigger than the tariff set by INA-CBGs. This study aims to find out the average inpatient treatment cost for the chronic kidney disease with hemodialysis and to find the most significant component that influences the hospital's tariff; the factors influencing the total treatment cost and the difference between real cost and INA-CBGs's tariff. This study was a cross-sectional analytical research with the perspective of hospital and retrospective data gathering method. The subject of this study was chronic kidney disease in-patients treated with hemodialysis who met inclusive criteria. The subject in this research had 47 inpatient episodes. The patient characteristic analysis employed descriptive statistics, the factors influencing the hospital cost analysis used Spearman correlation, and the tariff differences analysis employed one sample t test. The findings showed that on average, the real inpatient treatment cost for chronic kidney disease with hemodialysis with operative treatment per patient per treatment episode was IDR. 23,732,520.02 (SD IDR. 19,142,379.09) and with non-operative treatment was IDR. 12,800,910.61 (SD IDR. 6,409,290.00). In operative treatment category, the most significant component was the operative medical treatment cost as much as IDR. 104,611,200.00 (29.39%). Meanwhile, in non-operative treatment category, the most significant component was the medical support service cost as much as IDR. 111,085,001.00 (27.12%). Further, the factors influencing the inpatient treatment cost was comorbid, frequency of HD and LOS. Therefore, the difference between real cost and INA-CBGs's tariff was in N-4-10-II Class I; N-4-10-II Class II; N-4-10-III; N-4-10-I Class I and the tariff difference was IDR. 225,632,939.96.
