PELAKSANAAN PENYEDIAAN BERKAS REKAM MEDIS RAWAT JALAN DI PUSKEMAS WONOSARI II

ABSTRAK Latar Belakang : Pelayanan kesehatan wajib menyediakan fasilitas yang diperlukan dalam rangka penyelenggaraan rekam medis. Kecepatan penyediaan berkas rekam medis ke klinik juga dapat menjadi salah satu indikator dalam mengukur kepuasan pasien. Kesiapan petugas dan ketersediaan alat-alat yang diperlukan serta tempat yang layak untuk proses pendaftaran adalah hal terpenting untuk mencapai kecepatan waktu untuk mendaftar pasien. bahwa pelaksanaan penyediaan berkas rekam medis pasien rawat jalan di Puskesmas Wonosari II masih kurang maksimal, dimana pada proses penyediaan berkas rekam medis masih mengalami keterlambatan. Tujuan : Mengetahui proses penyediaan berkas rekam medis rawat jalan, Mengetahui rata � rata waktu yang diperlukan untuk penyediaan berkas rekam medis, dan Mengetahui faktor-faktor apa saja yang mempengaruhi keterlambatan penyediaan berkas rekam medis. Metode Penelitian : Jenis penelitian yang digunakan pada peneliti ini adalah penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian yang digunakan adalah penelitian fenomenologi. Teknik pengumpulan data yang digunakan dalam penelitian ini adalah observasi, wawancara dan studi dokumentasi. Hasil : Proses penyediaan berkas rekam medis rawat jalan di Puskesmas Wonosari II yaitu petugas mendaftar pasien di TPP, petugas filing mencarikan berkas rekam medis di rak penyimpanan, setelah itu berkas didistribusikan sesuai poliklinik tujuan. Rata-rata waktu kecepatan penyediaan berkas rekam medis diperoleh hasil 10,9 menit. Waktu tercepat dalam penyediaan berkas rekam medis yaitu 3 menit, sedangkan waktu terlama penyediaan berkas rekam medis yaitu 19 menit. Faktor � faktor yang mempengaruhi lama penyediaan berkas rekam medis di Puskesmas Wonosari II meliputi faktor man yaitu kurangnya sumber daya manusia (SDM). Faktor methods yaitu ; terjadinya missfile, alur pendaftaran pasien, kesalahan pendistribusian dan berkas rekam medis sampai poliklinik tidak urut antrian. Faktor material yaitu perbedaan ukuran map berkas rekam medis lama dan baru. Kata kunci : Penyediaan berkas rekam medis, pendaftaran, distribusi, faktor keterlambatan.

ABSTRACT Background : Health services must provide the facility which is need in order to the medical record. Speed provision of medical record file to the clinic also can be the one of indicators in measuring patients satisfaction. Readiness officer and the availability of the necessary equipment as well as a decent place for the registration process is the most important thing to achieve speeds time to registration the patients. The implementation of the provision of medical record file outpatients in Wonosari II health center is still not maximal, where the process of providing medical record file is still delay. Objective: knowing the process of provide outpatient medical record file, knowing the average time required to provide of medical record file, and knowing the factors that influence delays in providing of medical record file. Methods : This research used descriptive research with a qualitative approach. The design used is phenomenological research. The method of collecting the data is used observation, interview and documentation. Results : the process of provide outpatient medical record file in Wonosari II medical center are the officer will registration the patients in TPP, filing officer searching the medical record file in the storage rack, after the file is distributed according to clinic destination. Knowing the average time speed of supplying medical record file is 10.9 minutes. The fastest time in the provision of medical record file is 3 minutes, while the longest time provision of medical record files is 19 minutes. Factors - factors that affect the long provision of medical record file in Wonosari Puskesmas II includes man factor is the lack of human resources ( HR) . Factors methods namely ; missfile occurrence , flow patient registration , distribution errors and medical record file until the clinic is not a queue sequence. Material factors , namely the size difference map medical record file of old and new . Keywords: Provision of medical record file, provision file, registration, distribution, the delay factor.
