PERANCANGAN MAP (FOLDER) DAN FORMULIR RAWAT JALAN DI UPT PUSKESMAS WONOGIRI 1

Latar Belakang: Desain pada map rekam medis dan formulir rawat jalan masih banyak kekurangan. Diantaranya dilihat dari aspek anatomi, heading dalam desain pada formulir rawat jalan di UPT Puskesmas Wonogiri 1, yaitu belum mencantumkan nomor halaman, nomor edisi, nomor revisi, status pasien, dan nomor rekam medis kepala keluarga. Dari aspek isi dapat dilihat bahwa item yang terdapat dalam formulir tersebut belum lengkap. Masih ada item yang diperlukan untuk Klinik gigi karena belum terdapatnya lembar odontogram untuk pelayanan Klinik gigi. Untuk Map Rekam Medis sendiri belum terdapat item apapun untuk mengidentifikasi data pasien di dalamnya. Tujuan:Merancang Map Rekam Medis,Formulir Rawat Jalan, dan Formulir Klinik Gigi Proses Perancangan: Perancang melakukan studi pendahuluan di UPT Puskesmas Wonogiri 1 dan menentukan akan melakukan perancangan Map, Formulir Rawat Jalan, dan Formulir Rawat jalan Klinik Gigi. Kemudian dilakukan identifikasi formulir yang telah ada di UPT Puskesmas Wonogiri 1 dengan wawancara kepada responden. Setelah mengetahui hal yang perlu diperbaiki, perancang membuat dua alternatif desain formulir kemudian diajukan kepada responden untuk dipilih dan diberimasukan. Desain terpilih dilakukan revisi dengan memperhatikan masukan dari responden. Setelah revisi, perancang menunjukkan kembali hasil desain terpilih yang telah direvisi kepada responden untuk disetujui. Hasil: Desain map rekam medis terpilih menggunakan bahan ivory 260 gram berwarna putih. Dengan ukuran A3+. Lalu formulir rawat jalan dan formulir Klinik gigi menggunakan kertas hvs 70 gram berwarna putih dengan ukuran A4. Kata Kunci: Map Rekam Medis, Desain Formulir, Formulir Gigi, Formulir Rawat Jalan

ABSTRACT Background: Design on the map of medical records and outpatient forms still have many inadequacies. These deficiencies are able to be viewed from the aspect of anatomy and heading in the design of the form of UPT Puskesmas Wonogiri 1outpatient which have yet to specify the page number, issue number, revision number, patient status, and medical record number of paterfamilias. From the aspect of the contents, the items which are contained in this form is not yet complete. There is still a necessary part for poly dental because it does not have the presence of odontogram sheets for dental services. The Map of Medical Record itself does not have any item which aimed to identify patient data. Objective: Designing Map of Medical Record, Outpatient Form and Poly Dental Form. Designing Process: The researcher did a preliminary study in UPT Puskesmas Wonogiri 1 and decides to do the design Map of Medical Record, Outpatient Form and Poly Dental Form. Then the next process is identify the form which was used in UPT Puskesmas Wonogiri 1 by interviewing the respondents. After finding out the part of the form that need to be corrected, the researcher created two alternative design of forms then submitted to the respondents in order to indentify the preference of the respondents and get some feedbacks. Selected design will be revised by using the considering from the respondents. After the revision, the researcher show the revised design result to the respondents for approval. Results: Design of the Map of Medical Record which was selected using 260 grams of white ivory material, with A3 + size. Then the form of outpatient and poly dental is using white hvs paper 80 grams with A4 size. Keywords: Map of Medical Record, Form Design, Dental Form, Outpatient Form
