SISTEM PENYIMPANAN DAN PEMROSESAN REKAM MEDIS TERKAIT STANDAR AKREDITASI KRITERIA 8.4.3 DI PUSKESMAS JETIS 1 BANTUL

Latar Belakang: Puskesmas wajib terakreditasi setiap tiga tahun sekali. Hal ini menunjukkan perlu adanya upaya evaluasi terhadap kegiatan persiapan akreditasi yang telah dilakukan agar lebih siap menghadapi penilaian akreditasi berikutnya. Salah satu kriteria yang dinilai dalam akreditasi puskesmas adalah terkait sistem penyimpanan rekam medis. Berdasarkan hasil pengabdian kerja yang peneliti lakukan pada bulan Agustus 2015, hampir setiap hari terjadi missfile di tempat penyimpanan rekam medis. Oleh karena itu peneliti tertarik melakukan penelitian dengan judul sistem penyimpanan dan pemrosesan rekam medis terkait standar akreditasi kriteria 8.4.3 di Puskesmas Jetis 1 Bantul. Tujuan Penelitian: Penelitian ini bertujuan mengetahui sistem penyimpanan dan pemrosesan rekam medis terkait standar akreditasi kriteria 8.4.3. Metodologi Penelitian: Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif dan rancangan fenomenologi. Hasil: Kompetensi dan jumlah petugas rekam medis tidak sesuai dengan perhitungan kebutuhan petugas rekam medis di Puskesmas Jetis 1 Bantul. Surat Keputusan tentang Pengelolaan Rekam Medis dan Standar Operasional Prosedur tentang Penyimpanan Rekam Medis tidak sesuai dengan format penyusunan dokumen akreditasi. Pelaksanaan kegiatan identifikasi pasien tidak sesuai dengan urutan kerja pada Standar Operasional Prosedur yang ditetapkan. Desain formulir identitas pasien belum dapat mengumpulkan data secara efektif dan efisien. Tidak terdapat prosedur tetap tentang pengkodean keluarga. Tracer yang digunakan tidak memuat data nama pasien dan tujuan rekam medis. Petugas tidak melakukan analisis kelengkapan rekam medis sesuai dengan Standar Operasional Prosedur yang telah ditetapkan. Penentuan masa simpan rekam medis dan pelaksanaan penyusutan tidak seimbang dengan luas tempat penyimpanan yang tersedia. Peralatan yang digunakan dalam sistem penyimpanan dan pemrosesan rekam medis sudah tersedia dengan baik. Anggaran yang digunakan dalam sistem penyimpanan dan pemrosesan rekam medis menggunakan dana dari puskesmas sendiri karena berbentuk Badan Layanan Umum Daerah. Kesimpulan: Sistem penyimpanan dan pemrosesan rekam medis terkait standar akreditasi kriteria 8.4.3 berdasarkan unsur manajemen men, materials, dan method belum sesuai. Background: Puskesmas must be accredited every three years. This shows that it need for evaluation of the accreditation preparation that have been done in order to be better prepare for the next accreditation assessment. One of the criteria considered in puskesmas accreditation is about medical records storage. Based on the results of work dedication in August 2015, almost every day there are missfiles in storage of medical records in Puskesmas Jetis 1 Bantul. It was the reason why this research with the title storage system and medical records processing related to accreditation standards of criteria 8.4.3 In Puskesmas Jetis 1 Bantul is done. Objective: This reserach aims to know about storage system and medical records processing related to accreditation standards of criteria 8.4.3. Method: This type of research is descriptive research with a qualitative approach and phenomenal researh design. Result: Competence and number of medical records practitioner are not accordance with the calculation of human resource needs in Puskesmas Jetis 1 Bantul. SK about Pengelolaan Rekam Medis and SOP about Penyimpanan Rekam Medis is not accordance with the format of the documents accreditation. The implementation of patients identification is not accordance with the fixed procedure that have been determined. Form design of patient's identity is not good because it is not able to gather data effectively and efficiently. There is no fixed procedure about coding of family. Tracer that have been used do not contain data the patient's name and destination of the medical record. The health information management practitioner did not do the analysis of the completeness of medical record in accordance with the specified fixed procedure. The shelf life of the medical records active and the implementation of the medical records depreciation is not balanced with the storage space available. Equipment are used in the system of storage and processing of medical records is already available. Budgets are used in storage systems and medical record processing using from the health center own budgets because BLUD. Conclusion: Storage system and medical records processing related to accreditation standard of criteria 8.4.3 based on element management of men, materials, and the method is not appropriate.
