ANALISIS BIAYA TERAPI PADA PASIEN PENYAKIT GINJAL KRONIS DENGAN ANEMIA YANG MENJALANI RAWAT INAP DI RSUP DR. SARDJITO YOGYAKARTA TAHUN 2015

Anemia merupakan komplikasi yang sering terjadi pada pasien penyakit ginjal kronis stadium akhir (stadium 3, 4, dan 5). Kompleksitas terapi pada pasien penyakit ginjal kronis dengan anemia ini memerlukan biaya yang tidak sedikit. Penelitian ini bertujuan untuk mengetahui gambaran pengobatan anemia pada pasien penyakit ginjal kronis, gambaran biaya terapi pada pasien penyakit ginjal kronis dengan anemia, serta faktor-faktor yang mempengaruhi besarnya biaya terapi pasien penyakit ginjal kronis dengan anemia yang menjalani rawat inap. Penelitian ini merupakan penelitian analitik cross-sectional, pengambilan sampel dilakukan secara retrospektif dengan metode purposive sampling dari rekam medik pasien dengan diagnosis utama anemia karena penyakit ginjal kronis yang menjalani rawat inap di RSUP Dr. Sardjito Yogyakarta pada tahun 2015 serta rekap biaya perawatan pasien di Instalasi Teknologi dan Informasi (INSTI). Sudut pandang yang digunakan adalah dari rumah sakit dengan menganalisis biaya medik langsung. Data dianalisis dengan menggunakan metode deskriptif sedangkan faktor yang mempengaruhi biaya total biaya terapi dianalisis menggunakan analisis non-parametrik Kruskal-Wallis. Hasil penelitian menunjukkan terapi anemia yang digunakan adalah transfusi darah (85,92%), asam folat (78,87%), vitamin B12 (15,49%), zat besi (4,23%) dan eritropoetin-alfa (1,41%). Rata-rata biaya terapi pada pasien penyakit ginjal kronis dengan anemia adalah sebesar Rp 21.292.643,01 ± 16.193.545,10 tiap kali rawat inap. Faktor yang mempengaruhi perbedaan besarnya total biaya terapi adalah usia (p = 0,007), kelas perawatan (p = 0,047), dan lama rawat inap (p = 0,000).

The complexity of therapy in chronic kidney disease patients with anemia requires a large amount of cost. This study aims to describe the therapy of anemia in chronic kidney disease patients, the cost of therapy in chronic kidney disease patients with anemia, as well as the factors that affect the cost of therapy in hospitalized patients with chronic kidney disease and anemia. This study is a cross-sectional analytic, samples were taken retrospectively using purposive sampling from patients� medical records diagnosed with anemia due to chronic kidney disease who are hospitalized in RSUP Dr. Sardjito Yogyakarta in 2015, as well as patients� hospital bill collected from Instalasi Teknologi dan Informasi (INSTI). Perspective used in this study is from provider perspective or hospital perspective by analyzing direct medical costs. Data were analyzed using descriptive method, while factors affecting total cost of therapy were analyzed using non-parametric analysis Kruskal-Wallis test. The results showed that the most-used anemia therapy in chronic kidney disease patients were blood transfusion (85,92%), folic acid (78,87%), vitamin B12 (15,49%), iron (4,23%), and erythropoietin-alpha (1,41%). The average cost of therapy in hospitalized chronic kidney disease patients with anemia is Rp 21.292.643,01 ± 16.193.545,01 per patient. Factors affecting the total cost of therapy were age (p=0,007), hospital ward class (p=0,047) and length of stay (p=0,000).
