EVALUASI KESESUAIAN DAN KEAKURATAN KODE DIAGNOSIS PASIEN RAWAT INAP KASUS BEDAH ANTARA BERKAS KLAIM INA-CBG's DENGAN BERKAS REKAM MEDIS DI RSUD WATES

Latar Belakang: Penyelenggaraan rekam medis di RSUD Wates perlu diperhatikan karena sudah bekerja sama dengan BPJS. Berdasarkan 20 sampel awal didapatkan ketidaksesuaian kode diagnosis antara berkas klaim INA-CBG's dengan berkas rekam medis. Hal ini tentu berpengaruh pada keakuratan dalam laporan yang menjadi dasar dalam pengambilan keputusan dan berpengaruh terhadap mutu rekam medis. Dalam sistem pembayaran INA-CBG's, keakuratan kode sangatlah penting karena besarnya biaya bergantung dengan kode diagnosis akhir yang diberikan sesuai dengan ICD-10. Tujuan: Mengetahui pelaksanaan pengodean, menghitung persentase kesesuaian dan keakuratan kode diagnosis pasien rawat inap kasus bedah antara berkas klaim INA-CBG�s dengan berkas rekam medis serta faktor-faktor penyebab ketidakakuratan kode diagnosis. Metode: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian studi kasus. Metode pengambilan data dengan cara observasi, studi dokumentasi dan wawancara. Hasil: Pelaksanaan pengodean dilakukan oleh petugas pengodean dengan kualifikasi D3 Rekam Medis. Petugas pengodean berkas klaim INA-CBG's dilakukan dua petugas dan pada berkas rekam medis dilakukan satu petugas. Pelaksanaan pengodean pada berkas klaim INA-CBG's menggunakan software ICD-10 revisi 2008 dan berkas rekam medis menggunakan buku ICD-10 revisi 2010. Tingkat kesesuaian kode diagnosis antara berkas klaim INA-CBG's dengan berkas rekam medis sebesar 75%. Tingkat keakuratan kode diagnosis pada berkas klaim INA-CBG's yang sesuai ICD-10 sebesar 76,11%, sedangkan pada berkas rekam medis sebesar 73,89%. Faktor penyebab ketidakakuratan kode diagnosis adalah dokter, petugas pengodean dan prosedur tetap. Kesimpulan: Keakuratan kode diagnosis dipengaruhi oleh informasi diagnosis yang lengkap dan ketelitian petugas pengodean dalam memberikan kode. Kata Kunci: Diagnosis, Kesesuaian, Keakuratan, Kode

Background: Implementation of medical records in RSUD Wates need to be considered because it was cooperating with BPJS. Based on 20 samples, found that diagnosis code does not incompatibility the INA-CBG's claim file wih medical record file. It certainly affects the accuracy of the report is the basis for decision and influences the quality of medical records. In the payment INA-CBG's system, the accuracy of the code is important because of the cost depend on the final diagnosis code according to ICD-10. Objective: To examine the implementation of the coding, calculate the percentage of suitability and accuracy diagnosis code inpatient surgery cases between INA-CBG's claim file with medical record file, and the factors that cause of inaccuracies in diagnosis code inpatient surgical cases between INA-CBG's claim filewith medical record file. Methods: This type of research is descriptive research with a qualitative approach. Case study design. The method of collecting data by observation, documentation and interview studies. Results: Implementation of the encoding performed by personnel with qualifications encoding D3 Medical Record. Officers inpatient coding on INA-CBG�s claim file is done two people and the medical record file is done one person. Implementation of software coding on INA-CBG�s claim file using software ICD-10 in 2008 and the medical record file using the book ICD-10 in 2010. The ICD-10 diagnosis codes suitability between INA-CBG's claim file and medical record fileas much as 75%. The accuracy of diagnosis codes on INA-CBG's claim filea ccording ICD-10 as much as 76,11%. The accuracy of diagnosis codes on medicl record file according ICD-10 as much as 73,89%. Factors that cause inaccuracies in dignosis code is a doctor, cleck coding and standard procedure. Conclusion: The accuracy of diagnosis codes are affected by the specific or completeness diagnosis information and the accuracy of coding clerk in providing code.
