PERANCANGAN BERKAS REKAM MEDIS RAWAT INAP DI PUSKESMAS PANDAK 1 BANTUL TERKAIT PROSES KLAIM BPJS

ABSTRAK Latar belakang: Puskesmas Pandak 1 Bantul memiliki fasilitas pelayanan rawat inap yang mengakibatkan berkas rekam medis rawat inap menjadi sangat penting .. Namun dari studi pendahuluan yang telah dilakukan ditemukan bahwa berkas rekam medis rawat inap yang sudah ada masih butuh revisi dan salah satu formulir resume untuk proses pengajuan klaim BPJS tidak tersedia, hal ini membuat petugas mengajukan klaim dengan cara menfotocopy seluruh berkas rekam medis pasien. Proses fotocopy berkas rekam medis ini pun dilakukan diluar puskesmas sehingga berkas rekam medis pernah hilang saat di fotocopy. Tujuan: Mengetahui item – item data sosial dan medis pada berkas rekam medis rawat inap yang tepat untuk pengajuan proses kalim BPJS dan merancang formulir yang terdapat pada berkas rekam medis rawat inap dilihat dari aspek formulir. Metode: Teknik pengumpulan data yang digunakan dalam penelitian ini adalah observasi dan wawancara dengan instrumen penelitian berupa check list observasi dan daftar pertanyaan yang akan ditanyakan dalam penelitian ini. Hasil: Hasil dari penelitian ini berupa desain formulir rekam medis rawat inap yang lengkap dan dapat digunakan sebagai syarat proses kalim BPJS. Kesimpulan: Kesimpulan dalam penelitian ini dari dua alternatif pada tiap desain formulir yang ada dalam berkas rekam medis rawat inap akan dipilih satu alternatif .Disain yang paling cocok dengan proses kegiatan klaim yang akan dipilih. ABSTRACT Background: The inpatient services at Community Health Center Pandak 1 Bantul makes the medical record is very important. But from a preliminary study has been found that the inpatient medical records file of which there are still need of revision and one of the form of the resume to the process of filing of claim in is not available. This makes the submitting claims with the way copy the entire file record medical patients. The process of a copy file medical record this was done outside the community health center so that the medical records ever go away in a copy. Objective: Knowing the items of social data and medical data on the inpatient medical records file that is right for the submission of the process claim in and design the form contained in the inpatient medical records file from the aspect of form. Methods: The collection of data used in this research is observation and interviews with the research in the form of check list of observation and a list of questions that would be asked in this research. Results: are two of each design alternative forms that exist in the inpatient medical record file will have an alternative to any design that best matches the process of claims will be performed. Conclusion: The conclusion in this research is of two alternative of each design a form that is in the medical record stay will be selected one alternative in any design that best fit with the process of claim will be done.
