Latar Belakang : Perkembangan Ilmu teknologi yang semakin pesat dapat dimanfaatkan dengan positif salah satunya di bidang kesehatan. Puskesmas merupakan salah satu fasilitas pelayanan kesehatan tingkat pertama yang bertujuan untuk mencapai derajat kesehatan masyarakat yang setinggi tingginya. Puskesmas Semin II Gunungkidul Melakukan Upaya peningkatan kesehatan dengan program Kesehatan, salah satunya adalah Program Kartu Keluarga Sehat. Pelaksanaan pencatatan, pengolahan dan pelaporan kegiatan Kartu Keluarga Sehat masih menggunakan sistem manual. Masalah yang sering terjadi pada pelaksanannya adalah keterlambatan pengumpulan dan kesalahan rekap hasil pemantauan keluarga sehat. Sehingga keakuratan informasi yang didapat kurang dapat diterima. Hal tersebut memunculkan ide untuk merancang sistem informasi keluarga Sehat. Sistem Informasi keluarga sehat akan dibagi menjadi input system, database, dan output system yang akan dikerjakan oleh peneliti dan 2 peneliti lainnya. Dalam penelitian ini penulis berfokus pada Input system dari sistem informasi keluarga sehat Tujuan : Menganalisis Kebutuhan pengguna dan merancang antarmuka untuk Pendataan Sistem Informasi Keluarga Sehat Berbasis Web sesuai dengan Kebutuhan Pengguna. Metode : perancangan ini menggunakan metode penilitian action and developement. Dengan sistem kartu keluarga sehat Puskesmas Semin II Gunungkidul sebagai objek perancangan. Perancangan pendataan sistem informasi keluarga sehat ini didasari oleh observasi, studi dokumentasi dan hasil wawancara terhadap kader dan petugas puskesmas. Hasil : Berdasarkan hasil analisis dan wawancara kebutuhan pengguna, maka peneliti merancang Pendataan sistem informasi keluarga sehat Puskesmas Semin II Gunungkidul yang dapat mengoptimalkan Pengumpulan data secara cepat. Kesimpulan : kebutuhan sistem informasi keluarga sehat Puskesmas Semin II Gunungkidul mampu menyimpan data terkait identitas keluarga, indikator PHBS, Kadarzi, dan Rumah Sehat.

Background: The development of increasingly rapid technological science can be utilized with a positive developing, one of them in the health facilities. Primary Health Care (Puskesmas) is one of the first-level health care facility that aims to achieve a high degree of public health. Puskesmas Semin II Gunungkidul Efforts to improve health by Health program, one of the program is the Health Families Program Card. Implementation of this program is the recording, processing and reporting and still using manual systems. The problem that often occurs in the observance is to delay the collection and fault monitoring results recap healthy families. So that the accuracy of information obtained less acceptable. This led to the idea to design information systems healt family. health families Information systems will be divided into the input system, database, and output system that will be done by the researcher and two other researchers. In this study, the authors focused on system input of health family information systems. Objective: Analyze The user needs and designing interfaces for Data Collection Health Families Information System according to the Web, Based on User Needs. Method : This design uses action and development methods. With the health family system Puskesmas II Semin Gunungkidul as a design object. Data Collection design of the health family information systems is based on the observation, documentation study and interviews of the Kader and personnel health of Puskesmas. Results : Based on the analysis and interview the needs of users, the researchers designed a Data Collection of health family information system in Puskesmas Semin II Gunungkidul to optimize the collection of data quickly. Conclution : the needs of health family information system in Puskesmas Semin II is able to store data related to the identity of the family, PHBS Indicators, Kadarzi’s Indikators and health home’s Indicators.
