Analisis Pelaksanaan Sistem Penomoran Rekam Medis Rawat Jalan Di Rumah Sakit Khusus Bedah Islam Cawas

Latar Belakang: Sistem penomoran adalah sistem pemberian nomor rekam medis kepada pasien saat berobat di rumah sakit. Rumah Sakit Khusus Bedah Islam Cawas sudah menggunakan sistem penomoran unit, namun masih terdapat kejadian duplikasi nomor rekam medis dengan jumlah duplikasi sebanyak 5 berkas rekam medis rawat jalan selama satu minggu dari 269 berkas. Duplikasi yang terjadi adalah satu pasien memiliki dua nomor rekam medis atau lebih maka hal itu akan mempengaruhi kesinambungan data pasien. Tujuan: Menganalisis pelaksanaan sistem penomoran rekam medis rawat jalan di Rumah Sakit Khusus Bedah Islam Cawas. Metode: Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif. Rancangan penelitian adalah fenomenologi dengan pengumpulan data menggunakan teknik wawancara, observasi dan studi dokumentasi. Subjek penelitian ini adalah 5 petugas pendaftaran dan 1 kepala sub bagian rekam medis. Objek penelitian ini adalah sistem penomoran dan 373 berkas rawat jalan selama satu minggu pada pertengahan bulan Maret. Hasil: Pelaksanaan sistem penomoran rekam medis rawat jalan di Rumah Sakit Khusus Bedah Islam Cawas secara otomatis dari komputer dengan menggunakan sistem penomoran unit. Kendala-kendala terkait pelaksanaan berdasarkan unsur manajemen sumber daya manusia yaitu petugas tidak mengetahui SPO dan petugas kurang teliti saat mengidentifikasi pasien dan pasien lupa membawa KIB, berdasarkan unsur manajemen metode yaitu SPO yang belum sesuai dan belum terdapat SPO penetapan nomor rekam medis tunggal pada pasien yang memiliki nomor rekam medis ganda, dari unsur manajemen mesin yaitu fasilitas SIMRS yang belum lengkap dan duplikasi nomor rekam medis sebanyak 10 berkas dari 373 berkas rekam medis di bagian filing. Kesimpulan: Pelaksanaan sistem penomoran yang digunakan di Rumah Sakit Khusus Bedah Islam Cawas adalah sistem penomoran unit. Kendala-kendala terkait sistem pelaksanaan berdasarkan unsur manajemen terdiri dari unsur man, machine dan method.

Background: The numbering system is a system of medical record number to the patient during treatment at the hospital. Rumah Sakit Khusus Bedah Islam Cawas already using a numbering system unit, but still there are events duplication medical record number by the amount of duplication as much as 5 outpatient medical record file for one week of 269 files. Duplication that occurs is one patient had two or more medical record number and it will affect the continuity of patient data. Objective: To analizing implementation of the numbering system of outpatient medical records at the Rumah Sakit Khusus Bedah Islam Cawas. Method:This type of research is descriptive qualitative approach. The study design is the collection of data using the phenomenological interview, observation and documentation study. The subjects were 5 registrar and 1 sub section head of medical records. The object of this study is the numbering system and files 373 outpatients for one week in mid- March. Results: Implementation of the numbering system of outpatient medical records at the Rumah Sakit Khusus Bedah Islam Cawas automatically from the computer by using a numbering system unit. Constraints related to the implementation based on the elements of human resource management that the officer did not know the SPO and the attendant lack of thorough when identifying the patient and the patient forgot to take KIB, based element management methods, namely SPO is not appropriate and there has been no SPO determination of medical record number single patient have multiple medical record number, of elements of the machine such as facility management SIMRS incomplete and duplicate medical record number as many as 10 files out of 373 files on the filing of medical records. Conclusion: Implementation of the numbering system used at the Rumah Sakit Khusus Bedah Islam Cawas is the numbering system unit. Constraints related to implementation based on the element management system consisting of elements of man, machine and method.
